A Clinical Social Worker is integral to delivering comprehensive and integrated care. We are seeking a Licensed Clinical Social Worker to join our team that wants to be an integral part of an interdisciplinary care team (IDT) changing lives every day. We are passionate and purpose driven to be best-in-class. Our goal is to help our members live their best life based upon their personal goals. PACE allows us to invest in the right things to address root cause and environmental factors, not just symptoms and resulting medical care. Role Compensation: This is a mid-level role with room for advancement.  We are open to Part time or Full Time as the Program expands. Salary range is $52,000 -$72,800. We are expanding in multiple areas and you can grow with us. The Role Mission: The mission of the Licensed Clinical Social Worker is to serve as a member advocate and participate in the IDT (interdisciplinary care team). Below are key duties of the Licensed Clinical Social Worker. Promotes and maintains the mental and social health of enrolled participants through assessment, treatment, education, and counseling. Provides basic casework and consultation and facilitates communication between participants, their families, and the PACE program interdisciplinary team (IDT) with an objective to create dialogue among stakeholders Maintains responsibility for the implementation of the social work care plan and coordination of social work with other services to optimize participant care. Completes assessments at admission and for quarterly care planning according to regulatory requirements, and as condition change indicates. Determines participant and family needs related to social support, financial support, counseling, and housing. Confers with participant and family to identify participant goals and expectations. Coordinates with the IDT to develop a comprehensive care plan for each participant. Provides individual and group counseling to participants and their families as needed or as prescribed in the care plan. Acts as a participant advocate and liaison between the participant, family, IDT, and Care Team by facilitating family meetings, the Participant Council, and/or family support groups, education, or training sessions. Leads routine Family Caregiver meetings to provide education, support, and dialogue. Coordinates the completion of the participant's health care wishes and advance directives in cooperation with their primary care physician, the participant, and their family. Provides discharge planning in the event of disenrollment. Facilitates communication between the participant and various government programs such as Medicaid, SSI, Medicare, and Social Security. Reviews Medicaid eligibility, monitors time frame for recertification and facilitates Medicaid applications for certification and recertification. May participate in inter-agency meetings as needed. Assists participants in obtaining housing and eligibility for low-income housing options. Evaluates the need for and assists with the setup of money management systems for participants who require assistance. Maintains current knowledge of changing rules and regulations regarding Medicaid and Medicare eligibility and other entitlement programs and services. EDUCATION: Masters Degree LICENSURE/CERTIFICATION: Licensed Clinical Social Worker or Licensure in Process EXPERIENCE: At least 1 year experience in a hospital, nursing home, or community-based services. At least 1 year professional experience working with an elderly or frail population. Demonstrated understanding of psychosocial problems of aging. The ability to effectively work within an interdisciplinary team The ability to work effectively with culturally, economically, and educationally diverse populations The ability to form positive interpersonal relationships with a wide range of staff and clients. Desired or Preferred Knowledge, Skills and/or Abilities: At least 2 years' experience, post-Masters education. Demonstrated knowledge of mental health, especially in the elderly population. Experience with and knowledge of various government support services and programs, especially Medicaid and Medicare.

Job Summary: Social Workers/Case Managers supports guests staying at Water Street Mission by facilitating all aspects of guest case management, from intake through shelter and/or the Residential program. Social Workers/Case Managers work with guests from a strengths-based, person-centered, and trauma-informed perspective. Social Workers/Case Managers foster positive partnerships and collaboration with other WSM departments, as well as with external community resources providers, to maximize the time that guests have at WSM to contribute to their journey of sustainable restoration. In compliance with the PA Social Workers, Marriage and Family Therapists and Professional Counselors Act of 1987, amended in 2008 by PA Public Law 929, No. 68, only a person who holds a current license under the Act or who has received a bachelor's, master's, or doctoral degree from an accredited school or program of social work or social welfare may use the title “Social Worker.” Staff with relevant qualifications for this position but without social work credentials will be titled “Case Manager.” This distinction is in title only and does not constitute a separate job description or role function. FLSA:  Full-Time, Exempt All Water Street Mission staff must be: · Willing to sign the Water Street Missions' Statement of Faith · A current and active member and/or regular attendee of a local church. · Able to fully support the Water Street Mission's core values and ministry philosophy. Coordinate all functions of guest case management so that who we serve and how we serve is a true expression of our purpose, mission, and values. Provide case management to assigned Family Emergency Shelter, Providence Shelter, and Residential program guests on caseload, meeting with guests according to frequency designated by program. Complete guest intakes, shelter contracts, assessments, and monthly Residential guest Action Plans. Work with guests to identify their strengths-based, person-centered goals and make appropriate referrals to community service providers. In partnership with the Directors of Social Services and Behavioral Health Services and others as necessary, screen incoming referrals from behavioral health or substance abuse centers for intake eligibility, making recommendations for support plans as needed. Work positively and collaboratively with all staff to ensure coordinated services for guests. Attend and proactively contribute to all assigned internal and external meetings and trainings, including but not limited to Social Services team meetings, multidisciplinary and internal support team meetings, external community services provider meetings, etc. Maintain thorough and up-to-date knowledge of all WSM philosophies, policies, and procedures. Maintain or develop expertise in specific areas of social services, as assigned by Director. Maintain accurate and timely guest information and records. Assist with training and acclimating new Social Services department staff, as directed. Assist designated staff with responding to incidents or crises on campus, if requested. Provide coverage for Access Center desk as needed. The program diverts participants from jail or prison by conducting clinical assessments and then connecting them with long-term treatment in the community, along with rigorous judicial monitoring. The court aims to strengthen the justice system’s ability to identify, assess, and monitor individual participants, to create linkages between the justice and mental health systems, and to improve public safety by ensuring that participants receive high quality community-based services.   The Brooklyn Felony ATI Program utilizes a similar model to Brooklyn Mental Health Court, serving individuals facing felony charges who are not otherwise eligible for Mental Health Court or other treatment courts in Brooklyn. The program receives referrals through two pathways: (1) the dedicated Felony ATI court part within Brooklyn Supreme Court, with a dedicated judge and supervising prosecutor; and (2) through other court parts throughout Brooklyn Criminal and Supreme Court. Regardless of the referral pathway, all participants receive individualized assessments, onsite and community-based services, compliance monitoring and judicial monitoring for the life of the case.  The Center for Justice Innovation is seeking a Social Worker for its Brooklyn Felony Diversion programs. The Social Worker will serve individuals who are mandated to Felony Alternative to Incarceration (Felony ATI) programming through the Brooklyn Supreme Court. Reporting to the Clinical Director, the Social Worker is responsible for conducting psychosocial assessments, linking clients to a range of wrap-around services, providing comprehensive trauma-informed case management, compliance monitoring, and reporting for clients referred to the Brooklyn Mental Health Court and Brooklyn Felony ATI programs.  Responsibilities include but are not limited to:   Conduct comprehensive psychosocial clinical assessments, draft reports, and provide recommendations to court parties for services and treatment;  Provide clinically-informed case management services to participants that address issues such as substance use, mental health issues, trauma, domestic violence, housing, and educational/vocational needs; and other challenges in client’s life (e.g. new arrests, family court cases and other civil legal issues);  Manage and monitor compliance with treatment mandates, including care coordination with providers and obtaining compliance updates for court;  Work to ensure the provision of high-quality, evidence-based, and culturally responsive clinical assessments and treatment services across the site;  Provide written and verbal participant reports to the court and other court staff (e.g. prosecutors, defense attorneys), as needed;   Coordinate and collaborate with a consulting psychiatrist;   Work with the Clinical Director and interdisciplinary team to develop project specific policies, procedures, and data collection;  Support clients who present to the clinic in crisis or with imminent needs to be addressed;  Conduct intakes and needs-based screenings for newly mandated clients;  Provide written treatment updates for court stakeholders for all court-involved participants;   Coordinate with court stakeholders on any non-compliance issues and assist clients in getting back into compliance, including re-engaging clients, arranging return on warrants, and implementing gradual responses to encourage future compliance;   Maintain appropriate client information in electronic databases and paper files;   Make appropriate referrals to community-based organizations for additional services;  Establish and maintain linkages with community partners who can provide appropriate interventions and services to court-involved individuals;   Attend court on calendar days to help clients navigate court and collaborate with court staff and stakeholders, as needed;   Provide coverage in court by reporting on cases; as needed  Accompany clients to appointments in the community, as needed;  Participate in regular interdisciplinary case conferences with court stakeholders and treatment providers;  Provide clinical guidance and information on trauma-informed practices to Brooklyn Felony ATI Diversion team members;  Assist with the development and implementation of new initiatives; including best clinical practices, and appropriate evidence-based interventions;   Assist with administrative responsibilities including data collection and management, and grant reporting requirements, as needed;  Attend all required staff meetings and trainings;   Attend community-based events after hours, as needed; and Additional relevant tasks, as needed.  Qualifications: LMSW with a minimum of 4 years in criminal justice and/or a related field. A Clinical Social Worker is integral to delivering comprehensive and integrated care. We are seeking a Licensed Clinical Social Worker to join our team that wants to be an integral part of an interdisciplinary care team (IDT) changing lives every day. Role Compensation: This is a mid-level role with room for advancement. We are open to Part time or Full Time as the Program expands. Salary range is $52,000 -$72,800. We are expanding in multiple areas and you can grow with us.

The Role Mission:

The mission of the Licensed Clinical Social Worker is to serve as a member advocate and participate in the IDT (interdisciplinary care team). Below are key duties of the Licensed Clinical Social Worker.

  • Promotes and maintains the mental and social health of enrolled participants through assessment, treatment, education, and counseling.
  • Provides basic casework and consultation and facilitates communication between participants, their families, and the PACE program interdisciplinary team (IDT) with an objective to create dialogue among stakeholders
  • Maintains responsibility for the implementation of the social work care plan and coordination of social work with other services to optimize participant care.
  • Completes assessments at admission and for quarterly care planning according to regulatory requirements, and as condition change indicates.
  • Determines participant and family needs related to social support, financial support, counseling, and housing.
  • Confers with participant and family to identify participant goals and expectations.
  • Coordinates with the IDT to develop a comprehensive care plan for each participant.
  • Provides individual and group counseling to participants and their families as needed or as prescribed in the care plan.
  • Acts as a participant advocate and liaison between the participant, family, IDT, and Care Team by facilitating family meetings, the Participant Council, and/or family support groups, education, or training sessions.
  • Leads routine Family Caregiver meetings to provide education, support, and dialogue.
  • Coordinates the completion of the participant's health care wishes and advance directives in cooperation with their primary care physician, the participant, and their family.
  • Provides discharge planning in the event of disenrollment.
  • Facilitates communication between the participant and various government programs such as Medicaid, SSI, Medicare, and Social Security.
  • Reviews Medicaid eligibility, monitors time frame for recertification and facilitates Medicaid applications for certification and recertification.
  • May participate in inter-agency meetings as needed.
  • Assists participants in obtaining housing and eligibility for low-income housing options.
  • Evaluates the need for and assists with the setup of money management systems for participants who require assistance.
  • Maintains current knowledge of changing rules and regulations regarding Medicaid and Medicare eligibility and other entitlement programs and services.

EDUCATION: Masters Degree

LICENSURE/CERTIFICATION: Licensed Clinical Social Worker or Licensure in Process

EXPERIENCE:

  • At least 1 year experience in a hospital, nursing home, or community-based services.
  • At least 1 year professional experience working with an elderly or frail population.
  • Demonstrated understanding of psychosocial problems of aging.
  • The ability to effectively work within an interdisciplinary team
  • The ability to work effectively with culturally, economically, and educationally diverse populations
  • The ability to form positive interpersonal relationships with a wide range of staff and clients.

Desired or Preferred Knowledge, Skills and/or Abilities:

  1. At least 2 years' experience, post-Masters education.
  2. Demonstrated knowledge of mental health, especially in the elderly population.
  3. Experience with and knowledge of various government support services and programs, especially Medicaid and Medicare.
