What you will do Coordinates a clinical treatment program for the diagnosis, treatment or referral of clients/patients presenting moderate to severe social adjustment problems. Adheres to policies which assure the confidentiality of client communications. Provides for a multi-disciplinary, holistic approach to the treatment of clients, coordinating same with appropriate hospital departments. Conducts individual, family, or group therapy sessions in accordance with accepted principles of the National Association of Social Workers. Assesses progress of clients/patients and modify or extend treatment program accordingly. Coordinates care of clients/patients with referring physician or agency. Documents therapy in accordance with hospital policies and procedures on medical records. Intervenes as a crisis counselor in situations requiring immediate psychological treatment such as substance abuse, suicide attempts, psychotic episodes and other instances as outlined in hospital policy. Refers client or patient to outside agencies where appropriate. Communicates with family members/significant others in order to further the effectiveness of client/patient treatment and progress. Coordinates client/patient billing and modify treatment program in accordance with insurance coverage, plan provisions, and individual ability to pay. Provides educational programs on a variety of psychological topics to hospital/community groups as requested. Essential Qualifications The individual must be able to work the hours specified. A Brief Overview

The LPN practices under the supervision of a registered nurse, advanced practice registered nurse, licensed physician, or other health care provider. The LPN organizes and directs those aspects of care for which he/she is accountable; communicating and coordinating care with well-defined nursing diagnoses, including all health team members.

What you will do

  • Collects data and conducts a focused nursing assessment of the health status of a patient.
  • Participates with other health care providers in the development and modification of the patient-centered health care plan.
  • Implements nursing interventions within a patient-centered health care plan.
  • Assists in the evaluation of responses to interventions.
  • Provides for the maintenance of safe and effective nursing care rendered directly or indirectly.
  • Advocates for the best interest of the patient.
  • Communicates and collaborates with patients and members of the health care team.
  • Assists with health counseling and teaching.
  • Delegates and assigns nursing interventions to implement the plan of care.
