$19.75 - $30.00

Position Highlights

Monday- Friday 8:30am-5:00pm; 80 hours 2 weeks

No weekends/holidays.

(Occasionally evenings can be later if doing home visits 5:30pm)

Additional preferences:

  • CHW certification

  • Experience in helping clients apply for and access community/government services: ie- financial assistance, Medicaid, Social Security, SNAP

  • Spanish speaker preferred!

A Brief Overview

Accountable for serving as a liaison between health/social services and the community; understanding, developing and maintaining relationships with the community they serve to improve the quality and cultural competence of service delivery.

What you will do

  • Health system navigation and resource coordination, such as helping a patient find a provider, make an appointment for a service, arranging transportation to a medical appointment, attending an appointment with the patient for a medical service or helping a patient find other relevant community resources such as a support group.
  • Health promotion and coaching providing information or education to patients that makes positive contributions to their health status, such as cessation of tobacco use, reduction in the misuse of alcohol or drugs, improvement in nutrition and physical fitness, family well-being, control of stress and pregnancy and infant care including prevention of fetal alcohol syndrome.
  • Health education to teach or promote (content of the education must be consistent with established or recognized healthcare standards) methods and measures that have been proven effective in avoiding illness and/or lessening its effects.
  • Provide culturally appropriate health information and education to patients, such as assist with providing language translation services for patients who do not speak English.
  • Provide culturally relevant health education regarding social determinants of health that may impact the patient (such as cultural foods, traditions, etc.), and ensure family members also understand health information and education provided to the patient.

Required Education, License/Certification, or Work Experience:

  • High School or GED Equivalent
  • Community Health Worker- Certification Board Certification must be completed within the applicable state guidelines.
  • Drivers License- Licensing Board

Preferred Education, License/Certification, or Work Experience:

  • Experience with navigating local medical and social support systems.
