Assess tolerance of activity levels, collaborate with other team members, and make recommendations/referrals regarding appropriateness of return to work or ADLs. Complete ongoing patient assessments as determined by patient condition. Provide information to family and/or significant individuals in assessment activities and patient progress and condition. Perform initial assessments and initiate communication with healthcare team members regarding treatment planning; i.e. nutrition, psychosocial, medications, etc. Collaborate with staff and appropriate resource persons to formulate and update individual treatment plans (ITPs) to work toward positive patient outcomes.
