Apply Job Type Full-time Description The Case Manager's primary responsibility is to educate patients with their chronic disease, including behavioral health diagnosis, through support and oversight. Responsible for developing plan of care, service coordination, and linkages to key community services.10. Serves as liaison between patients and staff.11. Completes TB reporting, including medication management for patients who test positive and require public health reporting12. Patient nurse visits, including TB and Chronic Disease Education.13. Description

The Case Manager's primary responsibility is to educate patients with their chronic disease, including behavioral health diagnosis, through support and oversight. In addition, this position will also provide case management to patients with complex medical/behavioral health illnesses. Nurse Case Manager supports providers and supports staff through an integrated approach to care management and community outreach. Community outreach, such as home visits and health screenings will be required. Essential Functions:1. Promotes teamwork and enhances a positive work environment. Responsible for developing plan of care, service coordination, and linkages to key community services.10. Serves as liaison between patients and staff.11. Completes TB reporting, including medication management for patients who test positive and require public health reporting12. Patient nurse visits, including TB and Chronic Disease Education.13. Maintain HIPAA compliance at all times14. May triage patients by phone and/or walk-ins. 15. Other duties as assigned.
