Coordinates the exchange of medical and social information necessary to ensure continuity of care. Provides counseling to patients, as needed, to assist them in decision making and acceptance of illness and related problem, e.g. Advance Directives, adjusting to disfiguring illness, permanent disabilities. Develops and maintains new contacts with outside agencies in order to be used as a referral source. Coordinates financial referrals and counsels patients and their families informing them of various assistance programs and community resources. Adheres to all standards and policies regarding safety/patient safety initiatives. Collects and maintains necessary data for grant reporting and provides in a timely manner. This is a grant funded program that works with pregnant women affected with opiate. Some travel involved - doing some home visits. Hours vary but generally 8a-4:30p, Monday through Friday.

Primary Function/General Purpose of Position

The social worker assists patients and families to resolve all aspects which may prohibit or limit the patient from receiving maximum benefit from medical care. Evaluates the social-emotional functioning of patient and families, providing counseling when necessary and facilitates the discharge plan. Works cooperatively with other hospital departments, physicians, and outside agencies. Provides assistance to neonates, pediatric, adult, and geriatric patients.

Travel involved. Work with clients including home visits. Hours vary but generally 8a-4:30p, Monday through Friday.

Essential Job Functions

  • Conducts interviews with patients and families to obtain social work assessment and evaluation relevant to medical problems and discharge plan. Collaborates with entire health care team to ensure maximum benefits of medical care.

  • Completes social history and documents, when necessary, on patients.

  • Evaluates data and refers patients to appropriate community resources, and if necessary, facilitates placement. Coordinates the exchange of medical and social information necessary to ensure continuity of care.

  • Provides counseling to patients, as needed, to assist them in decision making and acceptance of illness and related problem, e.g. Advance Directives, adjusting to disfiguring illness, permanent disabilities.

  • Develops and maintains new contacts with outside agencies in order to be used as a referral source.

  • Coordinates financial referrals and counsels patients and their families informing them of various assistance programs and community resources.

  • Participates as a team member in multi-disciplinary conferences.

  • Maintains Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and department standards.

  • Adheres to all standards and policies regarding safety/patient safety initiatives.

  • Collects and maintains necessary data for grant reporting and provides in a timely manner.

  • Embraces Standards of Behavior in order to provide and promote excellent customer service for both internal and external customers. Holds self and others accountable for behaviors that promote service and excellence.
