SPECIFIC RESPONSIBILITIES: · Provide direct clinical consultation, crisis intervention and counseling services that are trauma informed, culturally responsive, flexible and attuned to the needs and circumstances of the individual resident and their family. · Effectively engage families and provide services to include parent coaching, family counseling, and parent support groups. Participate in all family conferences. Work with residential and medical staff to create and maintain open partnerships with families to support communication and visiting. · Work with team members to provide families with ongoing support to learn about trauma informed care and provide psychoeducation, counseling and coaching so that family caregivers can strengthen their own coping strategies and self-care and support, to increase their capacity to respond and support the child in a trauma responsive. · Participate in multidisciplinary treatment team conferences to formulate and implement treatment plans. The Position: Under the general supervision of the Social Work Supervisor, the Social Worker is a mental health professional, helping the frail and elderly PACE participants find solutions to a wide range of social problems, serves as an integral member of an Interdisciplinary Team (IDT) and is responsible for performing psychosocial assessments, developing and implementing plans of care, and facilitating communication between the participant, family and CEI staff. The hourly rate for the Social Worker role at Center for Elders Independence is $ 37.97 to $56.95 per hr. Hourly rate based on the market for the Social Worker position, as well as experience, skills, ability and work history Duties and Responsibilities: Serves as an interdisciplinary team (IDT) member responsible for developing and implementing a comprehensive, integrated plan of care, monitors progress toward objectives by evaluating behavioral adjustments and communicates participant changes to team members. Follows up on distress screenings, providing appropriate emotional support and referring to ancillary resources as needed. 3. Facilitates conversations surrounding advance care planning and advance directives, end of life issues, nursing home/assisted living transitions, palliative and hospice care. 4. Screens patients at risk for financial toxicity. Provides and assists patients with required applications to apply for patient assistance programs as deemed appropriate. 5. Assists patients in navigating insurance options when there is a loss of coverage or patient is uninsured or underinsured. 6. Serves as a patient advocate to access federal, state, and local resources identified to address financial and/or medical needs. 7. Documents all financial assessments, interventions, outcomes, referrals, and services provided. 8. Communicates and collaborates with the pharmacy, reimbursement, and nursing departments to minimize duplication of effort/services and ensure a seamless coordination of services. 9. Engages in continuous education and training to maintain skills, knowledge, and competency to complete functions of role. 10. Communicates and interacts with peers and other departments in a matter that is consisent with CHC's Standards of C.A.R.E. Interacts effectively patients/family members and demonstrates sensitivity in regards to cultural diversity, age specific needs and confidentiality. 11. All other duties as assigned.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities) 1. Must have a master’s degree from a school of social work accredited by the Council on Social Work Education. (LSW, LMSW, LCSW) 2. Minimum of one year's social work experience in health care setting. 3. Experience in a hospice care preferred. 4. Demonstrates good verbal and written communication, and organization skills. 5. Must have an active social work license in the state of Nevada. 6. The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job. Must have and maintain a valid driver’s license, maintain automobile insurance coverage and have access to an automobile. The Royal Borough Of Kensington and Chelsea Council is looking for an Adult Social Worker to join their Mental Health team, offering a pay rate of £32 per hour.

What will your responsibilities be?
In your role, you will provide a statutory social work Care Act assessment service to people with mental health problems, which includes case management, safeguarding adults, and initial preventative work. Additionally, you will carry out the statutory duties under the Mental Health Act and act as a Best Interests Assessor under the Deprivation of Liberty Safeguards (DoLS) provisions of the Mental Capacity Act, where you are qualified to do so.
