Looking for a forward thinker ready to lead a growing Social Services Team. This position will support successful Behavioral Health operations of Minnesota Community Care. They will work alongside the Director of Behavioral Health and other key stake holders to ensure that behavioral, mental health and social services across all sites match the needs of the patients. Direct Clinical Care Assess clinical symptoms, developmental and personality disorders, severity of psychosocial stressors, and address medical conditions as they affect patient functioning. Develop treatment plans based on individual client needs and resources. Provide a continuum of short- and long-term therapy services for individuals, families and groups based on treatment plans. Maintain comprehensive and current knowledge of community resources; consult with other clinic staff regarding these resources; make referrals as appropriate. Know, understand, and adhere to organizational policy related to the patient's rights for confidential care Provide assessment of crisis situations, and design and carry out appropriate interventions in crisis situations; coordinate and distribute mental health coverage schedules to clinic director. Document patient/family status and history, diagnosis, treatment plan, results and progress in medical record in a timely manner. Leadership Responsibilities Coordinate mental health team meetings along with Head of Behavioral Health; set agenda, document and distribute minutes. Provide consultation expertise to clinic staff and other mental health providers; coordinate and facilitate case reviews with mental health team. Coordinate grant reporting and responsibilities; direct staff and obtain outcome measurements and reporting of data. Work with leadership, clinical departments, and population health programs to participate in strategic planning, lead implementation of appropriate programs and initiatives in alignment with organizational objectives, and align behaviors Work with MCC stake holders to align behavioral health care across the system Coordinate the management of Behavioral Health risk Support strategic development of Behavioral Health care mode, identifying population needs, existing resources, and prioritizing gaps of care to be addressed. Support efforts to develop virtual and digital health initiatives addressing behavioral health needs Lead care management resources across the organization Lead efforts, alongside Head of BH to integrate evidence based clinical guidelines, preventative guidelines, protocols and other metrics in the development of treatment plans that promote clinical quality and efficiency in the delivery of healthcare for defined populations. Develop systems of care alongside Head of BH that monitor member health status and promote early interventions. Implement systems of care that facilitate close monitoring of high-risk members to prevent and/or intervene early during acute exacerbations. Manage utilization and practice metrics to further refine the delivery of care model to maximize clinical, quality, and fiscal outcomes for defined populations. Work with leadership to continuously evaluate process, identify problems and propose process improvement strategies to enhance delivery of care models Build strong relationships with patients, providers, clinicians, and other team members to promote care coordination Supervision and Management: Provides direct supervision Manager of Mental Health Case Management, including supervision of the ARMHS and Creando Puentes, BSW Student Interns and mental health programs. Ensure appropriate behavioral health staffing that report through the Director of Behavioral Health , including guiding hiring, orientation, coaching and performance reviews of team members who report to them. Responsible for maintaining and updating processes and procedures (training manuals, and other operational procedures and tools.) of the teams that report through them. Maintain professional affiliations, enhance professional development, and help to facilitate external relationships to keep current in the latest behavioral health care trends and to develop positive coordinated care efforts. Respond alongside Manager of Behavioral Health Operations in addressing escalated client issues/inquiries and delivering solutions. May Serve as part of the behavioral health leadership, participating in meetings and decision making. May supervise Student interns includes MSW students, LGSW students, or BSW student interns. This role requires regular walking. This role may come into contact with patients who may have contagious illnesses. Physical Demands Prolonged periods of sitting at a desk and working on a computer. Responsibilities and Initiatives To help NHF meet its growth goals, the Social Services Coordinator will: Timely complete Needs Assessment and Care Plans. Complete referrals and coordinate services for mental health, housing, etc. Facilitate discharge process and locating housing for individuals. Complete discharge notes, communicating to our community partners. Conduct warm hands off for guest when appropriate. Maintain up-to-date CRM proprietary database daily and partner contacts. Complete Coordinated Entry System ("CES") Documents and all other documents required to maintain compliance. Complete Homeless Management Information Systems (HMIS). Utilize bed board application to determine census capital for operational planning (i.e.) meal planning. Provide emergency service referrals (e.g., domestic violence, human trafficking & sexual assault) and warm handoff referrals when appropriate. Attend care coordination meetings. Collaborate with the nursing team to execute weekly case conferencing for guests. Practice harm reduction practices while conducting guests searches for contrabands or illegal substances when applicable. Support program team with appropriate guest interventions such as de-escalation, Crisis Prevention, motivational interviewing, etc. Engage and conduct daily check-in with guests, building rapport and trust with our guests. Ability to adequately manage caseload of a minimum 25 guests with flexibility to manage more to meet the needs of the facility (i.e., vacancies, PTO etc.). Flexibility to complete home visits when necessary to track post-transition guest progress and or Rapid Rehousing Housing (RRH) when applicable. Must adhere to administrative hours to complete all required documentation. Project a positive professional image and adheres to organizational dress code. Ability to multi-task and collaborate with external case managers to provide adequate services and support to the guests. Ability to triage and intervene during areas of guest behavioral concerns. Adhere to all funders and company’s standards procedures of operations, regulations, and best practices to meet compliance. Flexibility to adapt to schedule changes when necessary to meet the needs of the facility. Scheduled Hours 40 Position Summary This position partners with The SPOT/Project ARK Medical Directors on providing the strategic leadership for Project ARK (AIDS/HIV Resources and Knowledge) and The SPOT (Supporting Positive Opportunities with Teens) and is responsible for the supervision and management of all programs, staff and services. Position is responsible for the overall direction, development of policies and procedures, timeline, completion and financial outcome of projects pertaining to Project ARK and the SPOT. This position is responsible for maintaining compliance with the contract requirements for federal grants including the Ryan White Part D, SAMHSA and other federal, state, local and private grants received by Project ARK and The SPOT. The position will be the lead grant writer for the federal continuation and competitive renewal applications. The position is responsible for developing program evaluation procedures, completing progress reports, data reporting tables and other program evaluation information required by the funding sources. The position will maintain current program budgets and monitor all Project ARK and The SPOT sub awards. Project ARK/The SPOT have 4,286 Clients; 61 Staff (10 of which are supervisors); 11 Clinicians, as well as 2 clinicians and 8 staff with off-site collaborators. The Operating Budget for Project ARK is $2.8M; the SPOT is $1.7M. Job Description Primary Duties & Responsibilities: Operations Management Partners with The SPOT/Project ARK Medical Directors on providing the strategic vision and leadership for the program's services and staff in alignment with the School of Medicine's mission. Convenes regular leadership meetings and provides individual and team supervision. Mission driven: Serves as a visible advocate and spokesperson for the organization and its mission, and guides the organization as it engages in strategic decision making. Provides leadership and oversight to all aspects of The SPOT/Project ARK daily operations, programs, and staff. Directly supervises Project ARK and The SPOT Program Managers. Supports active performance management of staff and offers tools (continuing education and training) to inspire high performing teams. Serves as Principal Investigator/Project Director for the HRSA Ryan White Part D grant and 340B program. Responsible for grant deliverables and completion of project outcomes pertaining to Project ARK and the SPOT. Collaborates with all relevant groups to develop and manage project timelines to meet grant deliverables and to provide the highest quality of services possible. Primary point person for communications with all federal, state, and local Project Officers as well as collaborating institutions/foundations/outreach organizations. Meets regularly with St. Louis and Missouri and Illinois Ryan White grantee planning groups to ensure the coordination of HIV care and prevention resources and services within the region. Meets with other youth serving agency leaders to partner on the creation of new strategies to address the population needs. Disseminates project communications to all teams/groups as appropriate. Co-leads the Ryan White C/D Internal Strategic Planning Committee comprised of Program and Medical Directors and Quality Manager. Manages financial resources in partnership with the Project ARK Senior Grants Manager and DOP fiscal and administrative teams including SPA, OSRS, JROC etc., to meet all contractual requirements. Responsible for the writing and submission of grant/contract proposals in coordination with the program managers of the various programs of Project ARK/The SPOT. Creates, designs and implements innovative new HIV care and prevention programs and youth services. Presents on best practices at national, state and local conferences to further the care and treatment of our populations. Publishes best-practice findings and supports replication efforts across other Ryan White and youth programs. Responsible for the oversight of program evaluation and Clinical Quality Management requirements of the program and directly supervises the Part C/D Quality/Data Manager. Partners with the QM on completing progress reports, data reporting tables and other program evaluation information required by the funding sources. Completes all progress reports, data reports and other program evaluation information required by funding sources. Prepares for successful site visits and submission of progress reports. Partners with WUSM Development Office and Saint Louis Children's Hospital Foundation on meeting with and educating potential funders. Maintains professional public relations and handles complaints. Coordinates with WU Public Affairs related the community impact. Manages personnel within legal requirements and the policies of WU, which includes recruitment, hiring, terminations, training, professional development, supervision, and evaluation of personnel in alignment with the Department of Pediatrics guidelines. Financial / Grants Management Works with the Senior Grants Specialist to prepare, monitor, and manages all grant budgets related to Project ARK and the SPOT in order to meet program goals and expend funds within contract requirements. Monitors processing of contracts and submission of accurate and timely invoices to ensure fiscal contract deliverables are met. Responsible for maintaining compliance with HRSA and SAMHSA grant requirements including the 340B program requirements. Participates in the Washington University Part C/D 340B Oversight committee and other 340B leadership meetings. Works in coordination with the 340B coordinators and other staff members to ensure monitoring of income and expenditures in compliance with HRSA requirements. Provides oversight of subcontracts which includes communicating expectations and timelines, ensuring deliverables are on time, with no quality issues. Key point person for all grant management and programmatic fiscal deliverables and communications. Job Description

Looking for a forward thinker ready to lead a growing Social Services Team. This position will support successful Behavioral Health operations of Minnesota Community Care. They will work alongside the Director of Behavioral Health and other key stake holders to ensure that behavioral, mental health and social services across all sites match the needs of the patients. This role is 60% clinical and 40% administrative.

Direct Clinical Care

  • Assess clinical symptoms, developmental and personality disorders, severity of psychosocial stressors, and address medical conditions as they affect patient functioning.
  • Develop treatment plans based on individual client needs and resources. Provide a continuum of short- and long-term therapy services for individuals, families and groups based on treatment plans.
  • Maintain comprehensive and current knowledge of community resources; consult with other clinic staff regarding these resources; make referrals as appropriate.
  • Know, understand, and adhere to organizational policy related to the patient's rights for confidential care
  • Provide assessment of crisis situations, and design and carry out appropriate interventions in crisis situations; coordinate and distribute mental health coverage schedules to clinic director.
  • Document patient/family status and history, diagnosis, treatment plan, results and progress in medical record in a timely manner.

Leadership Responsibilities

  • Coordinate mental health team meetings along with Head of Behavioral Health; set agenda, document and distribute minutes.
  • Provide consultation expertise to clinic staff and other mental health providers; coordinate and facilitate case reviews with mental health team.
  • Coordinate grant reporting and responsibilities; direct staff and obtain outcome measurements and reporting of data.
  • Work with leadership, clinical departments, and population health programs to participate in strategic planning, lead implementation of appropriate programs and initiatives in alignment with organizational objectives, and align behaviors
  • Work with MCC stake holders to align behavioral health care across the system
  • Coordinate the management of Behavioral Health risk
  • Support strategic development of Behavioral Health care mode, identifying population needs, existing resources, and prioritizing gaps of care to be addressed.
  • Support efforts to develop virtual and digital health initiatives addressing behavioral health needs
  • Lead care management resources across the organization
  • Lead efforts, alongside Head of BH to integrate evidence based clinical guidelines, preventative guidelines, protocols and other metrics in the development of treatment plans that promote clinical quality and efficiency in the delivery of healthcare for defined populations.
  • Develop systems of care alongside Head of BH that monitor member health status and promote early interventions.
  • Implement systems of care that facilitate close monitoring of high-risk members to prevent and/or intervene early during acute exacerbations.
  • Manage utilization and practice metrics to further refine the delivery of care model to maximize clinical, quality, and fiscal outcomes for defined populations.
  • Work with leadership to continuously evaluate process, identify problems and propose process improvement strategies to enhance delivery of care models
  • Build strong relationships with patients, providers, clinicians, and other team members to promote care coordination

Supervision and Management:

  • Provides direct supervision Manager of Mental Health Case Management, including supervision of the ARMHS and Creando Puentes, BSW Student Interns and mental health programs.
  • Ensure appropriate behavioral health staffing that report through the Director of Behavioral Health , including guiding hiring, orientation, coaching and performance reviews of team members who report to them.
  • Responsible for maintaining and updating processes and procedures (training manuals, and other operational procedures and tools.) of the teams that report through them.
  • Maintain professional affiliations, enhance professional development, and help to facilitate external relationships to keep current in the latest behavioral health care trends and to develop positive coordinated care efforts.
  • Respond alongside Manager of Behavioral Health Operations in addressing escalated client issues/inquiries and delivering solutions.
  • May Serve as part of the behavioral health leadership, participating in meetings and decision making.
  • May supervise Student interns includes MSW students, LGSW students, or BSW student interns.

Supervisory Responsibilities

Works independently within scope of the job description with general direction and consultation from supervisor. Is responsible for supervision of Case Managers, social services, and students interns as appropriate. May provide work direction to other staff in consultation with leadership. May supervise and/or direct contractors, volunteers, and/or students.

Work Environment

Primary environment is home office, administrative office, or clinical office. This role requires regular walking. This role may come into contact with patients who may have contagious illnesses.

Physical Demands

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift 15 pounds at times.

Travel Requirements

Must have the ability to travel between MCC sites in the St. Paul/Minneapolis metro area.

Required Education and Experience

  • Master's level degree in Psychology, Social Work, or related field along with at least 5 years relevant experience.
  • Strong understanding and/or experience of providing behavioral health care.
  • Minimum 3 years of experience providing care, preferably in a community health center setting.
  • Lived experience relevant to expanding access for integrated behavioral health services for marginalized, underserved people may be substituted for required professional experience.
