About the Role The Social Worker plans, organizes and implements social work services to participants and their caregivers in accordance with Seen Health policies and all applicable regulations. Work with the primary care physician and other members of the care team to guide smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home, etc.). Acts as a resource to other team members and staff regarding topics such as dementia, difficult behaviors, and difficult personalities. Supports the IDT in establishing behavior management plans. Opportunity Summary:

This internship experience will provide an opportunity to work with a range of patient populations while developing professional and interpersonal skills needed to become a competent social worker.

Time Frame:

This opportunity will run from September 2025 - April 2026. 

Learning Model: 

Students will be under the supervision of a licensed social worker and will learn through observation, demonstration, and ongoing feedback to ensure that each individual’s learning needs can be met fully and in an individual manner.  Self-reflection, self-evaluation, and peer critique will further help to advance professional growth and development.
