Summary/Objective This position interacts directly with client families during their time of need and is responsible for creating and maintaining a premier level of client family satisfaction. Your Role as a Physical Therapist

In this role, you will contribute to our mission by delivering high-quality physical therapy services and supporting optimal patient outcomes. Key responsibilities include:

  • Promoting Elara Caring's philosophy, mission, and administrative policies to ensure consistent, high-quality care
  • Providing direct patient care, including functional evaluations, treatment planning, and outcome assessment
  • Collaborating with interdisciplinary care teams and consulting with other specialists as needed
  • Designing and implementing individualized plans of care based on comprehensive patient assessments
  • Adjusting care plans as patient needs evolve to maximize functional independence
  • Directing and supervising support staff to help patients achieve functional goals
  • Maintaining accurate, timely documentation of evaluations, treatment plans, progress notes, and required signatures
  • Acting as a clinical resource and providing leadership to promote physical therapy services within the community

  • Physically demanding and fast-paced clinical environment
