As such, these individuals experience very high behavioral health and physical health risks, which have not been successfully addressed by traditional forms of outpatient treatment. The primary function of ACT Teams is to help reduce unnecessary hospitalizations and incarcerations, by providing individuals the resources essential to thrive and maintain independence in their natural environment. POSITION SPECIFIC DUTIES & RESPONSIBILITIES: Maintain a strong working knowledge of the core ACT principles and ensure clinical practices reflect a high standard of model fidelity. Attend and participate in daily organizational meetings and weekly treatment team meetings with multi-disciplinary team, utilizing a team approach in working with ACT clients. Provide routine medication delivery and support to clients in community. Promote healthy lifestyles, including general wellness education and information specific to chronic medical conditions. Have good working knowledge of community resources for basic needs such as food, shelter, and clothing. Refer and link ACT clients as needed. Provide clients with psychosocial rehabilitation services to help support successful functioning in the community, including the areas of ADL's, recreation, education, employment, and family functioning. QUALIFICATIONS: Case Managers for ACT programs must have a Bachelor's degree in Social Work, Psychology or a related social services major, which qualifies for Case Management II certification with the Oklahoma Department of Mental Health. Candidates must have CM-II certification or obtain CM-II certification within 60 days of employment. Applicants with a previous background working in community mental health, inpatient psychiatric facilities, criminal justice, home health, or similar settings are preferred. Strong candidates with a passion for helping individuals with severe mental illness will also be considered. Must have a valid driver's license and satisfactory driving record; use of personal automobile will required for travel to locations outside of primary office. Lead, monitor and manage processes for the OCCR team as required for annual data submission for the registry to obtain and maintain NAACCR Gold Certification standards. Create files of uploads, consolidations, sequence 90 cases, and duplicate cases to distribute among OCCR staff. Oversees death clearance process including running the death match program, reviewing and updating results, linking with hospital discharge data, creating follow back file and monitoring the status of hospital and non-hospital facilities, and adding death certificate only cases to the main database. Assist with the preparation and submission of data for each annual Call for Data including monitoring and managing processes, as required, to maintain the highest data quality standards set forth by NAACCR and CDC NPCR. Oversight for quarterly reporting compliance including monitoring reporting facilities on complete, timely and quality of data submissions to ensure facilities are in compliance. Assist Registry Program Manager in tracking and oversight of compliance remediation plans for delinquent facilities.   • Identify customer's needs and provide realistic assistance in selection of home
  • Obtain credit information and work with manager to secure financing for customer
  • Obtain customer's down payment prior to ordering the home
  • Close contract with the customers
  • Complete and manage orders with customers
  • Utilize follow up systems to track activity and results
  • Follow-up on sales leads from various sources including advertising, telephone and website
  • Identify local marketing opportunities for new business
  • Proactive follow up with prospective customers
  • Availability: Must be able to work evenings, weekends and some holidays

MINIMUM QUALIFICATIONS

  • At least 2 years experience in sales or business
  • High School Diploma required; Bachelors Degree preferred
  • Management background is a plus
  • Bilingual skills is a PLUS
