You will directly engage, manage, and triage complex clinical needs. Typical Duties Include Developing a proper understanding of chronic kidney disease to provide optimal, coordinated population health care. Proactive patient management via text using the Phamily platform. Proactive patient management via phone calls. Triage and assessment of patient health status. Coordination of care between patients and providers. Active review of patient charts to assess health needs and gaps. Providing a positive patient experience for patients and families through courteous digital interactions. Routing patient escalations accurately and expeditiously.
