OCCUPATIONAL THERAPIST | DETAILS AND COMPENSATION:

• Location: Fayetteville WV 25840 – 100% on-site
• • Counties covered: Fayette and Nicholas counties
• Payrate: $60.68/hr + $0.54/mile
• Required Availability: Full-Time | Monday - Friday, 8am-5pm (No on call or Weekends required)
• • Benchmark requirement of 30 points per week.

OCCUPATIONAL THERAPIST | SUMMARY AND HIGHLIGHTS:

The Occupational Therapist will provide comprehensive home-based care for patients in the Fayetteville area. This role focuses on helping patients achieve independence within their own environment through personalized care plans, safety assessments, and therapeutic interventions.

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

OCCUPATIONAL THERAPIST | RESPONSIBILITIES:

• Patient Assessment: Analyze patient care needs and establish functional goals for comprehensive plans of care, including discharge planning.
• Home Safety: Assess patient environments and support systems to ensure a safe and effective delivery of care.
• Clinical Documentation: Utilize OASIS documentation for progress tracking and ensure all clinical records meet agency and regulatory standards.
• Care Coordination: Act as a case manager to coordinate with other disciplines, physicians, and community resources to enhance patient outcomes.
• Education: Provide vital instruction to patients and caregivers regarding diagnosis, medications, infection control, and safety measures.
• Supervision: Provide clinical supervision for Occupational Therapy Assistants (COTAs) and Home Health Aides.

OCCUPATIONAL THERAPIST | REQUIREMENTS:

• Education: High School Diploma or GED (verifiable)
• Attendance: Commitment to mandatory attendance during the first 90 days.
• Licensure: Valid West Virginia (WV) Occupational Therapy License is required (Compact licenses are accepted).
• Experience: Previous Home Health experience is required.
• Proficiency in OASIS documentation is required
• Experience with Homecare Homebase (HCHB) software is highly preferred.
• Productivity: Ability to meet the benchmark requirement of 30 points per week.
• Travel: Must be willing to travel to patient homes within Fayette and Nicholas counties.

Physical & Safety Standards

• Proficiency in safe body mechanics to prevent injury.
• Strict adherence to universal precautions and infection control standards.
• Ability to sanitize and transport equipment used for patient care.
