Job Description
Role Summary
The Physical Therapist (PT) provides high-quality, patient-centered care within a hospital-based outpatient setting, supporting patients across the continuum of care. This role focuses primarily on outpatient rehabilitation while also assisting patients recovering from surgical procedures.
The PT evaluates, develops, and implements individualized treatment plans designed to restore mobility, reduce pain, improve function, and enhance overall quality of life. This position offers the opportunity to collaborate with physicians, surgeons, and an interdisciplinary healthcare team in a dynamic hospital environment.

Duties & Responsibilities
Perform comprehensive physical therapy evaluations for outpatient and post-surgical patients
Develop individualized treatment plans based on physician referrals and clinical findings
Implement evidence-based therapeutic interventions to improve strength, mobility, balance, endurance, and functional independence
Provide post-operative rehabilitation for orthopedic and other surgical patients
Educate patients and families on home exercise programs, injury prevention, and recovery strategies
Document patient evaluations, progress notes, and discharge summaries accurately and timely in the electronic medical record (EMR)
Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes
Collaborate with physicians, surgeons, nursing staff, and the broader healthcare team to coordinate care
Ensure compliance with hospital policies, safety standards, and regulatory requirements
Maintain a clean, safe, and organized treatment environment
Participate in quality improvement initiatives and departmental meetings
Perform other related duties as assigned
Required Qualifications
Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy from an accredited program
Current, active state Physical Therapist license
CPR certification (BLS required)
Strong clinical reasoning and evaluation skills
Ability to manage a primarily outpatient caseload with some post-surgical patients
Excellent communication and patient education skills
Ability to work independently and collaboratively within a hospital-based team
Proficiency with electronic medical records (EMR) systems
Desired Qualifications
Previous experience in a hospital-based outpatient setting
Experience working with post-operative orthopedic patients
Strong manual therapy skills
Experience with functional outcome measurement tools
Bilingual abilities (preferred but not required)
Location & Work Type
On-site position within a hospital-based outpatient rehabilitation department.
