The primary purpose of the CNA position is to provide patients with daily care in accordance with state and facility established nursing procedures and as directed by their Supervisor. Every effort has been made to make your job description as complete as possible. However, this in no way implies that these are the only duties you will be required to perform. General: Check daily assignments at the beginning of each shift. Complete vital signs within first hour of each shift and record them on the flow sheet. Inform RN of any significant changes in patient condition or any abnormal values. Check Rehab, school, and activity schedule for patients and plan day accordingly. Give bed bath/shower which includes oral care, shampoo as necessary, and ear and nail care. Assist with setting up patients for meals and feeding patients as appropriate. Change bed linens QOD. Change more frequently if needed. Reposition those patients every two hours who are unable to turn or reposition. themselves using proper body alignment. Apply/remove splints as instructed. Record accurate I/O on patients requiring I/O. Record accurate calorie counts on patients requiring careful nutritional assessment. Maintain running totals of I/O, keep vital signs recorded when taken and record % of meal consumed immediately after meals. Ambulate/assist patients with mobility as ordered. Make available to nurses one hour prior to end of shift tabulations of I&O’s and percentages of meals taken. Complete all documentation one hour prior to end of shift. Make sure to communicate with nurse regarding any questions or concerns pertaining to patients and their care. At 6am, 2pm and 10pm, make sure patients have fresh water as appropriate. Assist with Activity program as assigned. Only one person from each hallway may be on break at any given time and you need someone who is assigned to your hallway to relieve you. Notify nurse when you leave for a break and when you return and who is responsible for the care of your patients. Need to double check all suction canisters after emptying to make sure they function. B&B Documentation every two hours success or failure of each attempt Assist Licensed personnel when requested or needed. Must possess the ability to make independent decisions when circumstances warrant such actions. Must be able to deal tactfully with personnel, patients, family members and various other visitors to facility. Must be able to work harmoniously with professional and non-professional staff. Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents. Must be flexible and willing to incorporate new methods into existing nursing practices. Must not pose a direct threat to the health and safety of other individuals in the workplace. Works throughout the nursing service area. Attends and participates in continuing education programs (24 hours of Cont. Ed. Per year). Room Neatness: Wipe off over bed tables and bedside tables. All personal items put together and put away (basin in center bedside cabinet) medicated creams and ointments returned to licensed personnel. No extra linen stockpiled on beds or furniture. Wheelchairs in wheelchair building with their foot pedals on the wheelchair. Wipe off all equipment and IV poles. Vanderlift sling, urinals and graduates to go in bathroom racks and labeled. Maintain neat and orderly environment Provide patients with clean suction catheters and boats at 0400-1200-2000. Fill boats with sterile water. Empty mist drain bags q shift at 0400-1200-2000. Maintain bathrooms in clean and orderly fashion with all items labeled. Maintain supplies in rooms at stock levels as indicated. Replenish at 2am. Stock suction catheters at night (please make sure that each patient has the appropriate size in their drawer). See size posted on bulletin board. Keep hallways clear of furniture, wheelchairs, extra equipment. Always ensure that hospital is maintained in a clean and orderly fashion. This includes all ancillary department rooms (x-ray, lab, play room, rehab room, utility room, central supply), patient rooms and staff lounge. Fill large volume nebulizers with sterile H 2 O two times a shift. All beds and privacy curtains must be away from doors. Evening and night shifts make sure all doors are locked. Emergency preparedness in your patient rooms. Be sure that you know where a flashlight is located at the beginning of your shift. Neat wheelchair placement in wheelchair building. Safe Environment: Assure all patient rooms have flashlights available. Equipment is stored in a safe manner with plugs in red outlets as necessary. All beds and privacy curtains are away from doors. All outside doors and windows secured (locked) after 6 pm. Staff member is aware of all fire safety, disaster, evacuation, code information. Fire safety preparedness. Admission/Discharge Preparation: Set up rooms for admissions. Ensure appropriate supplies and equipment. Orient patient/family to room, hospital, laundry usage, call light etc. When a patient is transferred or discharged, it is your responsibility to clean the room. so remove patient’s belongings and take it to the nursing office. Return unopened supplies to Central for credit. Discard loose, opened supplies. Complete belonging sheet and have family sign on admission and when family brings additional items to facility. When admitting a patient make sure they are aware of hospital laundry usage and call light usage. Always introduce yourself. Be sure to complete clothing list and have patient or family to sign. Weight and height on admission. Salary Range: $23.00-$27.00/hour

Completes 24 hour report daily; Monitors residents' intake and output; Participates in the monthly medication renewal process; Completes forms used throughout the residents chart; Documents any exceptions to residents' condition, including daily charting on ill residents; 24-, 48-, or 72-hour follow-up charting on residents with infections; incidents or new admits; Interprets policies and procedures to personnel, residents, visitors, and government agencies as required; Ensures residents charge slips are completed and forwarded to the appropriate department for processing; Accurately maintains daily census reports and submits to business office daily, as applicable; Duties/Responsibilities: Develops, delivers/implements, revises, and evaluates individualized patient-centered care plans under the supervision of a physician Provides case management to an assigned group of patients and ancillary staff Understands and submits required data collection sets (OASIS, PAR, etc.) and other documentation as requested by the state and/or federal government and insurance payers within required timeframes Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs per state and payor requirements Acts as an advocate for patient welfare and coordinates care between patients, their families/caregivers and/or their authorized representative, the agency and other healthcare providers/facilities/outside agencies Furnishes those services requiring substantial specialized nursing skill according to competency level Demonstrates problem solving abilities and positive interpersonal communication skills Identifies care preferences of patients and verifies that interventions meet the patient’s needs and their goals of treatment Initiates appropriate preventive and rehabilitative nursing procedures Prepares clinical and progress notes for each patient visit and summaries Attends case conferences as assigned Informs physician and other personnel of changes in the patient’s condition and needs Counsels the patient and family/significant others in meeting nursing and related needs. Educates on the proper home health strategies and procedures utilizing community resources as applicable Observes and reports patient symptoms, reaction to treatments, drugs and changes in the patient’s physical or emotional condition, responsible for medication reconciliation Participates in in-service programs, supervising and teaching other nursing personnel as assigned Administers medication per agency policy and physician orders Provides written instructions for aides or other ancillary disciplines caring for the patient Processes orders and notifies physician of patient needs and changes in condition Refers cases with physician’s orders to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker for those patients requiring their specialized skills Understands and adheres to the Policy/Procedure, Best Practices, Infection Control Plan, EMS Plan and the Compliance Plan Participates in clinical record/utilization review of medical records and quality assurance and performance improvement (QAPI) quarterly and as assigned. Reports/records all applicable infections, occurrences, and complaints Submits documentation within 24 hours or per state requirements Maintains confidentiality of patient, employee, and agency matters Takes on-call duty nights, weekends, and holidays, as assigned. When functioning as the on-call RN acts as the after-hours supervising nurse and makes nursing assessment and triage decisions under the guidance of the overall Clinical Supervisor as applicable Immediately reports to Nursing Supervisor any patient incidents/variances or complaints Demonstrates competent performance of technical skills according to established procedures Maintains acceptable attendance status, per Agency policy Reports all incomplete work assignments to Nursing Supervisor Demonstrates effective time management skills through daily documentation and infrequent overtime for routine assignments Ensures that the following information/instruction is given in writing to the patient/caregiver/representative: Visit schedule with frequency of visits, complete Medication Profile, any treatments to be administered by agency staff (POC), any other pertinent instruction related to the patient’s care needs, and name and contact information of the agency’s Clinical Manager Performs all other duties as assigned Required Skills/Abilities/Knowledge: Average computer skills with competency in media communications Must have the physical abilities to perform the described duties Able to organize work procedures through time management, assume responsibility and effectively supervise ancillary disciplines per state requirements Able to secure transportation and travel as required Able to apply professional training to critically think through patient needs, follow physician’s orders and produce accurate records of nursing activity within agency guidelines Acceptance of philosophy and goals of this Agency Ability to exercise initiative and independent judgment Able to meet all health requirements for clearances and also for current CPR Able to maintain licensure and continuing education as applicable Excellent verbal and written communication Education/Experience/Licenses/Certifications: Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing One (1) year nursing experience and home care (preferred) Benefits + Perks of Joining the Team Select Family Medical, Dental, and Vision Insurance 401(k) Referral Program Flexible scheduling Paid, hands-on training Weekly pay Tuition Assistance Programs and Continuing Education opportunities Nursing programs for new graduates available!
