Full- Time Clinical Supervisor: Nursing Anticipated Coverage area: Placer County Area In this leadership role, reporting the Director of Nursing, you will be responsible for ensuring that patient care is coordinated and managed appropriately by a team of Registered Nurse (RN) Case Managers and LVNs. You will serve as the first point of escalation ensuring support is easily accessible in a timely fashion; You will participate in the identification, development, design, and implementation of nursing educational programs to support clinical excellence and determine success measures and evaluate the effectiveness of these programs to promote ongoing staff development as well as patient and customer satisfaction. You will also meet regularly with our customer to maintain relationships and provide updates such as readmission reports, offering insights to improve patient outcomes and reduce readmissions. This is a hybrid of field work and on site based at 21HHS office: Sacramento Office: Spend 2 days per week in the office for administrative and team management duties. Fieldwork: Spend 3 days per week (average 12+ visits/per week) in the field conducting supervisory visits and offering direct support to RN Case Managers What you'll do as the Clinical Supervisor - Nursing Responsible for in the field oversight of clinicians who provide care and treatment to patients in their place of residence. Daily patient activities: Handles incoming calls timely, and follows up as necessary to ensure appropriate actions are taken to meet the internal and external customer needs. Coordinates training and educational programs for nurses within the field to improve clinical outcomes and competencies. Assesses staff learning needs and formulates specific plans and goals for next evaluation period with employees, performs administrative call duties, and serves as a role model and resource for staff. Functions as the first point of contact for field clinicians to assist in patient care escalations etc. Coordinates at least bi-weekly Case Conferences with clinical team to discuss patients' progress and for revision of the care plan as needed to meet planned outcomes. Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate. Reviews patient’s clinical diagnosis, medications, procedures and clinical course. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a plan of care. Full- Time Clinical Supervisor: Nursing
Anticipated Coverage area: Placer County Area

In this leadership role, reporting the Director of Nursing, you will be responsible for ensuring that patient care is coordinated and managed appropriately by a team of Registered Nurse (RN) Case Managers and LVNs.  You will serve as the first point of escalation ensuring support is easily accessible in a timely fashion; You will participate in the identification, development, design, and implementation of nursing educational programs to support clinical excellence and determine success measures and evaluate the effectiveness of these programs to promote ongoing staff development as well as patient and customer satisfaction. You will also meet regularly with our customer to maintain relationships and provide updates such as readmission reports, offering insights to improve patient outcomes and reduce readmissions.

This is a hybrid of field work and on site based at 21HHS office:

Sacramento Office: Spend 2 days per week in the office for administrative and team management duties.

Fieldwork: Spend 3 days per week (average 12+ visits/per week) in the field conducting supervisory visits and offering direct support to RN Case Managers

What you'll do as the Clinical Supervisor - Nursing

    • Responsible for in the field oversight of clinicians who provide care and treatment to patients in their place of residence.
    • Daily patient activities: Handles incoming calls timely, and follows up as necessary to ensure appropriate actions are taken to meet the internal and external customer needs.
    • Coordinates training and educational programs for nurses within the field to improve clinical outcomes and competencies. 
    • Assesses staff learning needs and formulates specific plans and goals for next evaluation period with employees, performs administrative call duties, and serves as a role model and resource for staff. 
    • Functions as the first point of contact for field clinicians to assist in patient care escalations etc. 
    • Coordinates at least bi-weekly Case Conferences with clinical team to discuss patients' progress and for revision of the care plan as needed to meet planned outcomes.
    • Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate.
    • Reviews patient’s clinical diagnosis, medications, procedures and clinical course.
    • Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a plan of care.

To be a Success as the Clinical Supervisor - Nursing

    • Two years' management or supervisory experience within the past five (5) years in a health care setting, preferably in Home Health.
    • Graduate of an accredited school of nursing, Bachelor’s Degree preferred.
    • Current CA Registered Nurse License (RN) 
    • Current Basic Life Support (BLS) Card
    • Valid CA Drivers License with acceptable driving record All license and certifications must be current at time of hire and sustained throughout your employment
$143,000 - $163,000 a year
Wage range Clinical Supervisor: 143,000.00- 163,000.00 plus a generous annual bonus of $40,000.00, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements.  21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (Up to 4% match)
