Job Description:

Hospice Certified Nursing Assistant

Location: Jackson Territory

Schedule: Monday-Friday, day shift

What You'll Do

  • Provide hands-on, compassionate care following each patient's care plan

  • Offer comfort, dignity, and emotional support during end-of-life care

  • Assist patients and families in learning self-care skills

  • Report updates to the RN Case Manager and care team

  • Document care provided, changes in condition, and patient needs

  • Help prevent unnecessary hospitalizations and support continuity of care

What We're Looking For

  • High School Diploma or GED

  • 1+ year of experience as a Nursing Assistant or Hospice Aide

  • Completion of a state-approved Hospice Aide training program with competency evaluation (per 42 CFR §484.36)

  • Hospice or home health experience preferred

  • Reliable transportation, valid driver's license, and auto insurance (50% travel)

  • Preferred to live in the service area

  • Ability to lift 50-100 lbs and perform hands-on caregiving duties
