The Certified home health aide/Certified Nursing Assistant is a paraprofessional member of the home health care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient’s needs. The home health aide is responsible for observing patients, reporting these observations, and documenting observations and care performed.

The home health aide will be assigned in a manner that promotes quality, continuity, and safety of a patient’s care.

Full Job Description

We are looking for a compassionate Medicare-certified Home Health Care Aide who has worked for a previous Skilled Nursing Home Healthcare agency. The qualified candidate preferably has at least one year of working at a Medicare Certified skilled home healthcare agency; to provide assistance in daily living and personal care services in patient's homes in accordance with an established care plan.

Responsibilities

  • Provide health care services in patient's residences
  • Administer simple prescribed medications
  • Assist with client’s personal care activities
  • Monitor patients (vital signs, temperature, respiration, etc.) and report on their condition
  • Maintain patient care records and document provided services
  • Assist patients with mobility and physical therapies/exercises
  • Instruct and counsel patients and families on diet and exercise
  • Collect routine specimens
  • Provide companionship and basic emotional or psychological support

Skills

  • Proven working experience as a Licensed home health care aide
  • Ability to monitor vital signs and to collect specimens
  • Familiarity with basic nutrition and personal hygiene standards
  • Nursing and health care administration skills
  • Caring and compassionate personality
  • Current CPR certificate
  • First Aid training
  • Valid driver’s license
  • High school degree
