The BSN Program at VTSU is in the second year of transformation from legacy Castleton University and we are excited to grow our faculty complement with new leadership and organizational structure. This is an in-person faculty position that will require the individual to manage the on campus clinical skills lab as part of the workload. Clinical expertise will determine additional courses taught. There are several opportunities for new faculty to create a dynamic active learning environment for students. Faculty expectations are commensurate with the expectations outlined in the Faculty Federation Agreement and include demonstrating effective teaching and advising; engaging in scholarship and professional practice; and providing service to the University and the greater Vermont community. Qualified candidates will be offered a tenure track position or a one-year terminal contract, renewable for up to three years based on the individual's years of full-time clinical nursing experience and years of full-time teaching in an accredited nursing program. Full- Time Clinical Supervisor: Nursing Territory: Multiple Opportunities: San Francisco County to Redwood City In this leadership role, reporting the Director of Nursing, you will be responsible for ensuring that patient care is coordinated and managed appropriately by a team of Registered Nurse (RN) Case Managers and LVNs. You will serve as the first point of escalation ensuring support is easily accessible in a timely fashion; You will participate in the identification, development, design, and implementation of nursing educational programs to support clinical excellence and determine success measures and evaluate the effectiveness of these programs to promote ongoing staff development as well as patient and customer satisfaction. You will also meet regularly with our customer to maintain relationships and provide updates such as readmission reports, offering insights to improve patient outcomes and reduce readmissions. This is a hybrid of field work and on site based at 21HHS office: Burlingame Office: Spend 2 days per week in the office for administrative and team management duties. Fieldwork: Spend 3 days per week (average 12+ per week) in the field conducting supervisory visits and offering direct support to RN Case Managers. What you'll do as the Clinical Supervisor - Nursing Responsible for in the field oversight of Nurses who provide care and treatment to patients in their place of residence. Daily patient activities: Handles incoming calls timely, and follows up as necessary to ensure appropriate actions are taken to meet the internal and external customer needs. Coordinates training and educational programs for nurses within the field to improve clinical outcomes, producitivity and competencies. Assesses staff learning needs and formulates specific plans and goals for next evaluation period with employees, performs administrative call duties, and serves as a role model and resource for staff. Functions as the first point of contact for field clinicians to assist in patient care escalations etc. Coordinates at least bi-weekly Case Conferences with clinical team to discuss patients' progress and for revision of the care plan as needed to meet planned outcomes. Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate. Reviews patient’s clinical diagnosis, medications, procedures and clinical course. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a plan of care. Job Summary: The Caregiver provides direct personal care and related services in the home that are allowed to be performed under state law.  She/he functions under the direction, instruction and supervision of the RN Supervisor. 

 

Certified Nursing Assistant Responsibilities:

  • Provide direct patient care under the supervision of a registered nurse or licensed practical nurse.
  • Assist patients with activities of daily living, such as bathing, dressing, and grooming.
  • Help patients with mobility and transfers, including lifting and positioning.
  • Monitor and record vital signs, such as temperature, pulse, and blood pressure. 
  • Assist in the maintenance of a clean and safe environment for patients. 
  • Collaborate with healthcare professionals to ensure quality patient care. 
  • Communicate effectively with patients and their families to provide emotional support
  • Skills:
  • Ability to lift and move patients safely and with proper body mechanics.
  • Experience in assisted living or long-term care settings preferred.
  • Knowledge of patient care techniques and infection control protocols.
  • Strong interpersonal skills for effective communication with patients, families, and healthcare team members.
  • Understanding of social work principles for providing holistic care to patients.
  • Familiarity with dementia care practices and behavior management techniques.
  • Previous experience in homecare or related fields is a plus.

Certified Nursing Assistant Requirements:
