Summary The Medication Aide, under supervision of the charge nurse, provides assistance with distribution of medications to residents. Assists with CNA responsibilities as needed. The Long Term Care Certified Nursing Assistant (C.N.A.) provides direct care for the Long-Term Care residents that follows the Care Plan and assists each resident to achieve and maintain the highest level of health and physical condition. General Hours of Work Part time, Evening shift, every 3rd weekend and every 3rd holiday. Education and/or Experience Completion of 75 hour C.N.A. course and Certified Medication Aid required. Prefer high school diploma or GED. Prior related work experience preferred. Attends 12 hours of in-service training yearly. Competent on the use of blood glucose machine. Certificate, Licenses, Registrations Certified as a Nursing Assistant and Medication Aide in the State of Iowa required. Essential Functions Communicates effectively with residents, families, and coworkers to protect and promote each resident's dignity, individuality and rights. Assesses patient's daily needs and treatments. This includes obtaining and recording vital signs, height, weight, assessing need for assistance with or independence in dressing needs, bathing, eating, and ambulation. The CNA conducts the assessments in consideration for the patient's choice, privacy, dignity, and independence. As a member of the multidisciplinary team, they provide input in the care planning process. Provides or assists the patient with daily needs and treatments in accordance with established policy and procedure. This includes assisting patients with bathing and dressing needs, shaving, oral, hygiene, bed making, transferring into and out of bed, repositioning, toileting needs, serving and collecting food trays, encouraging and/or assisting with feeding, answering call lights, insuring a clean environment, transporting patients upon admission and dismissal, assisting with ambulation and orienting/reorientating of patients to the facility environment to insure patient safety. Performs or assists patients in performing specific medical care activities in accordance with established policy and procedure to contribute to the patient's plan of care. This includes assisting patients in performing restorative exercises, collecting specimens administering enemas, bowel and bladder training, assessing and documenting completed cares, intake and output, baths, dietary intake/calorie counts on flow sheets while providing important feedback to nursing staff immediately on status or changes in patient conditions, pain management, and other important concerns. Performs safety and infection control procedures to insure a safe, clean environment for patients and staff. This includes understanding and following established policy and procedures related to fire, tornado, evacuation, disaster plan, universal precautions, infection control procedures, hazardous and chemical communication program. Demonstrates knowledge of the strategic and operational plans of the facility and actively supports these on a daily basis and as a part of this involvement actively supports all facility functions. Must follow the CDC legal and ethical guidelines set by the state, resident's rights, and organization for the distribution of medication to residents, and must be knowledgeable about the medication or research information. Know the special physical conditions of the resident that might affect how medication would react. Receives work assignments from Charge Nurse including assisting with medication pass, obtaining vital signs and blood glucose readings, and other duties within scope of practice. Stays current on resident care plans, reviews report sheets and new interventions at beginning of shift and as needed. Observes residents and notes physical condition, attitude, reactions, appetite, etc. Acts as leader and mentor to fellow CNA team members. Reports any changes or unusual findings to Charge Nurse. Performs other duties and responsibilities as assigned.

Provides corrective instruction to nursing staff concerning quality of care and quality of life issues, and customer service. Assumes responsibility for unit or shift compliance with facility policy and procedure, rules, regulations and standards of practice. Assures that residents receive needed nursing care and services to meet their needs according to the established plans of care and physician's orders and instructions. Customer Service Interacts with residents, families, staff, and other visitors to the center in a pleasant, respectful, and courteous manner. Acts as a positive representative of the facility at all times. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the unit. Performs incidental housekeeping and maintenance tasks as may arise during the course of regular duties, in order to maintain a clean, safe, pleasant environment for residents, visitors and staff. Residents/Patients' Rights Provides privacy and maintains the confidentiality of all resident care/information Ensures care and security of residents' personal possessions. Reports all complaints and grievance made by residents, family members, and visitors to the supervisor. Ensures that residents are free from abuse (physical, mental, and sexual), mistreatment and neglect, and reports any such instances to appropriate facility staff. Medical Reporting Completes and distributes nursing reports to the Director of Nursing and the designate physician within the given time requirement. Reviews the 24-hour report. Follows up on outstanding actions to ensure completion prior to the end of shift. Provides upcoming shift nurses with the obtained resident report and any follow up or special care necessary. Documents in daily assignment report any orders given to nurses or assistants in regards to care of resident. Reviews electronic medical record (EMAR/ETAR) to ensure completion of medication administration, treatments and assigned care prior to the end of the shift. Ensure proper staffing ratios are in place in adherence to jurisdiction, federal and state requirements. Reviews all incident reports for completeness. Ensures new interventions are added as needed and that the documentation is accurate. Admissions Functions Performs review of charts and resident information with all new admissions to the Nursing Center or Assisted Living Units. Participates in/receives report from transferring facility of resident to be admitted to the care of Ingleside. Validates completion of admission assessments pertinent to resident care are completed. Verifies all orders with the current physician before placing order with the designated pharmacy. Follows community admissions procedures and communicates resident admission to all proper staff, physicians, and service providers. Summary

  • The Medication Aide, under supervision of the charge nurse, provides assistance with distribution of medications to residents. Assists with CNA responsibilities as needed.
  • The Long Term Care Certified Nursing Assistant (C.N.A.) provides direct care for the Long-Term Care residents that follows the Care Plan and assists each resident to achieve and maintain the highest level of health and physical condition.

General Hours of Work

  • Part time, Evening shift, every 3rd weekend and every 3rd holiday.

Education and/or Experience

  • Completion of 75 hour C.N.A. course and Certified Medication Aid required. Prefer high school diploma or GED. Prior related work experience preferred. Attends 12 hours of in-service training yearly. Competent on the use of blood glucose machine.

Certificate, Licenses, Registrations

  • Certified as a Nursing Assistant and Medication Aide in the State of Iowa required.

Essential Functions

  • Communicates effectively with residents, families, and coworkers to protect and promote each resident's dignity, individuality and rights.
  • Assesses patient's daily needs and treatments. This includes obtaining and recording vital signs, height, weight, assessing need for assistance with or independence in dressing needs, bathing, eating, and ambulation. The CNA conducts the assessments in consideration for the patient's choice, privacy, dignity, and independence. As a member of the multidisciplinary team, they provide input in the care planning process.
  • Provides or assists the patient with daily needs and treatments in accordance with established policy and procedure. This includes assisting patients with bathing and dressing needs, shaving, oral, hygiene, bed making, transferring into and out of bed, repositioning, toileting needs, serving and collecting food trays, encouraging and/or assisting with feeding, answering call lights, insuring a clean environment, transporting patients upon admission and dismissal, assisting with ambulation and orienting/reorientating of patients to the facility environment to insure patient safety.
  • Performs or assists patients in performing specific medical care activities in accordance with established policy and procedure to contribute to the patient's plan of care. This includes assisting patients in performing restorative exercises, collecting specimens administering enemas, bowel and bladder training, assessing and documenting completed cares, intake and output, baths, dietary intake/calorie counts on flow sheets while providing important feedback to nursing staff immediately on status or changes in patient conditions, pain management, and other important concerns.
  • Performs safety and infection control procedures to insure a safe, clean environment for patients and staff. This includes understanding and following established policy and procedures related to fire, tornado, evacuation, disaster plan, universal precautions, infection control procedures, hazardous and chemical communication program.
  • Demonstrates knowledge of the strategic and operational plans of the facility and actively supports these on a daily basis and as a part of this involvement actively supports all facility functions.
  • Must follow the CDC legal and ethical guidelines set by the state, resident's rights, and organization for the distribution of medication to residents, and must be knowledgeable about the medication or research information.
  • Know the special physical conditions of the resident that might affect how medication would react.
  • Receives work assignments from Charge Nurse including assisting with medication pass, obtaining vital signs and blood glucose readings, and other duties within scope of practice.
  • Stays current on resident care plans, reviews report sheets and new interventions at beginning of shift and as needed.
  • Observes residents and notes physical condition, attitude, reactions, appetite, etc.
  • Acts as leader and mentor to fellow CNA team members.
  • Reports any changes or unusual findings to Charge Nurse.
  • Performs other duties and responsibilities as assigned.
