What You'll Be Doing The Palliative Care Advanced Practice Provider (APP)—either a Nurse Practitioner (NP) or Physician Assistant (PA)—will serve as the primary palliative clinician for residents in a single skilled nursing facility in Griffin, Georgia. This role is ideal for a provider passionate about improving quality of life for medically complex, predominantly geriatric patients through expert symptom management, serious-illness communication, and coordinated, patient-centered care. This is a part-time position with significant flexibility . You may work a few hours each day, or opt for fewer, longer days without any overnight call. For APPs seeking full-time work, there is an opportunity to add an additional facility in the Metro Atlanta area. In this role, you will collaborate closely with the SNF care team—including attending physicians, nurses, social services, therapy teams, and families—to deliver timely, goals-aligned palliative care that enhances comfort, reduces unnecessary hospital transfers, and supports thoughtful care planning. Key Responsibilities Provide palliative care consultations and follow-up, including assessment, symptom management, treatment planning, and monitoring for conditions such as pain, dyspnea, anxiety, nausea, constipation, and other distressing symptoms. Lead goals-of-care and advance care planning discussions with patients and families; ensure appropriate documentation and alignment with treatment plans. Manage acute and chronic conditions in partnership with attending clinicians, emphasizing comfort, quality of life, and avoidance of unnecessary interventions. Evaluate changes in condition promptly and provide high-quality clinical assessments to support early intervention and avoid preventable hospitalizations. Collaborate with the facility's interdisciplinary team to develop, update, and communicate individualized care plans. Support care transitions, including post-hospital assessments, medication reconciliation, and family/caregiver education. Maintain thorough and compliant documentation of assessments, care plans, communication, and follow-up. Participate in quality initiatives and process improvements focused on serious-illness care and reducing acute-care utilization. Serve as a resource to facility partners on palliative care best practices and evidence-based symptom management. Skills & Abilities Strong clinical knowledge of adult and geriatric medicine, with particular expertise in symptom management for seriously ill patients. Excellent communication skills, with the ability to lead sensitive conversations around goals of care with empathy and clarity. Ability to collaborate effectively with patients, families, nurses, and facility leadership in a skilled nursing setting. Strong organizational and documentation skills, with the ability to manage a panel independently with physician support. Commitment to evidence-based practice and high-quality, patient-centered care. Qualifications Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Georgia. Certification appropriate to role: NPs: Adult-Gerontology, Family Practice, or Internal Medicine PAs: NCCPA certification At least 1 year of experience in palliative care (not hospice); experience in geriatrics or skilled nursing facilities strongly preferred. Certification in Hospice and Palliative Care preferred (or willingness to obtain within 12 months - paid by the practice) Unrestricted DEA license (or ability to obtain) preferred Experience working independently in a facility setting with physician oversight. Familiarity with EHR systems and clinical documentation best practices. What You'll Be Doing

The Palliative Care Advanced Practice Provider (APP)—either a Nurse Practitioner (NP) or Physician Assistant (PA)—will serve as the primary palliative clinician for residents in a single skilled nursing facility in Griffin, Georgia. This role is ideal for a provider passionate about improving quality of life for medically complex, predominantly geriatric patients through expert symptom management, serious-illness communication, and coordinated, patient-centered care.

This is a part-time position with significant flexibility. You may work a few hours each day, or opt for fewer, longer days without any overnight call. For APPs seeking full-time work, there is an opportunity to add an additional facility in the Metro Atlanta area.

In this role, you will collaborate closely with the SNF care team—including attending physicians, nurses, social services, therapy teams, and families—to deliver timely, goals-aligned palliative care that enhances comfort, reduces unnecessary hospital transfers, and supports thoughtful care planning.

Key Responsibilities

  • Provide palliative care consultations and follow-up, including assessment, symptom management, treatment planning, and monitoring for conditions such as pain, dyspnea, anxiety, nausea, constipation, and other distressing symptoms.
  • Lead goals-of-care and advance care planning discussions with patients and families; ensure appropriate documentation and alignment with treatment plans.
  • Manage acute and chronic conditions in partnership with attending clinicians, emphasizing comfort, quality of life, and avoidance of unnecessary interventions.
  • Evaluate changes in condition promptly and provide high-quality clinical assessments to support early intervention and avoid preventable hospitalizations.
  • Collaborate with the facility's interdisciplinary team to develop, update, and communicate individualized care plans.
  • Support care transitions, including post-hospital assessments, medication reconciliation, and family/caregiver education.
  • Maintain thorough and compliant documentation of assessments, care plans, communication, and follow-up.
  • Participate in quality initiatives and process improvements focused on serious-illness care and reducing acute-care utilization.
  • Serve as a resource to facility partners on palliative care best practices and evidence-based symptom management.

Skills & Abilities

  • Strong clinical knowledge of adult and geriatric medicine, with particular expertise in symptom management for seriously ill patients.
  • Excellent communication skills, with the ability to lead sensitive conversations around goals of care with empathy and clarity.
  • Ability to collaborate effectively with patients, families, nurses, and facility leadership in a skilled nursing setting.
  • Strong organizational and documentation skills, with the ability to manage a panel independently with physician support.
  • Commitment to evidence-based practice and high-quality, patient-centered care.

Qualifications

  • Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Georgia.
  • Certification appropriate to role:
    • NPs: Adult-Gerontology, Family Practice, or Internal Medicine
    • PAs: NCCPA certification
  • At least 1 year of experience in palliative care (not hospice); experience in geriatrics or skilled nursing facilities strongly preferred.
  • Certification in Hospice and Palliative Care preferred (or willingness to obtain within 12 months - paid by the practice)
  • Unrestricted DEA license (or ability to obtain) preferred
  • Experience working independently in a facility setting with physician oversight.
  • Familiarity with EHR systems and clinical documentation best practices.
