Participate in quality improvement initiatives and contribute to the rehabilitation department's success. Qualifications: Doctorate or Bachelor’s/Master’s degree in Physical Therapy from an accredited program. Current and valid Physical Therapist license in the state of Colorado. Experience in skilled nursing or rehabilitation settings is preferred. Strong communication and teamwork skills with a resident-centered approach. Ability to work independently and adapt to a flexible PRN schedule. Dedication to providing compassionate and effective therapy services. We are seeking a PRN Physical Therapist (PT) to join our dedicated rehabilitation team. This position offers flexibility and the opportunity to make a meaningful impact on residents' lives in a rewarding setting.

 

Position Summary:
As a PRN Physical Therapist, you will evaluate and treat residents to improve their mobility, strength, and functional independence. You will work collaboratively with a multidisciplinary team to deliver high-quality, individualized care.

 

Key Responsibilities:

  • Perform comprehensive evaluations of residents’ physical abilities and needs.
  • Develop and implement personalized treatment plans to improve strength, mobility, balance, and overall function.
  • Provide therapeutic interventions, including exercises, manual therapy, and training on assistive devices.
  • Monitor residents’ progress and modify therapy plans as needed to achieve goals.
  • Collaborate with an interdisciplinary team to ensure holistic care.
  • Educate residents and their families on therapy goals, exercises, and strategies for continued improvement.
  • Accurately document evaluations, treatment sessions, progress notes, and discharge plans in compliance with regulatory and facility standards.
  • Participate in quality improvement initiatives and contribute to the rehabilitation department's success.

 

Qualifications:

  • Doctorate or Bachelor’s/Master’s degree in Physical Therapy from an accredited program.
  • Current and valid Physical Therapist license in the state of Colorado.
  • Experience in skilled nursing or rehabilitation settings is preferred.
  • Strong communication and teamwork skills with a resident-centered approach.
  • Ability to work independently and adapt to a flexible PRN schedule.
  • Dedication to providing compassionate and effective therapy services.
