You will recommend and demonstrate strategies to help children adapt and succeed in the classroom and at home. Additionally, you will refer children for additional evaluation by partner agencies for Part B and Part C services and mental health consultation. You will document and track screenings, individualized services, and referrals in our child-tracking software. In this role, you will provide direct support to children and families as needed through Program Intervention Plans. You will also work with our Mental Health and Disabilities Coordinator to recommend referrals to partner agencies and provide assistance with Individual Education Plans (IEP) or Individual Family Service Plans (IFSP). Ability to plan, organize, coordinate, direct, and control all aspects of a new or renovation project. Ability to work with the customer to find mutually satisfactory solutions to difficult issues. Ability to work with management systems and a variety of professional, administrative and industry personnel. General working knowledge of major Mechanical Systems. Experience with Windows and MS Office; experience with COINS OA a plus. Demonstrated skill at developing and nurturing client relationships. Excellent communication skills - verbal and written. Ability to organize and lead a group of people. Ability to work independently or as member of a team. Ability to adapt to ever changing priorities.   • Wichita, KS and surrounding areas
  • Pay Range: $65,000 - $90,000

As a Phoenix RN, you will be the first point of contact between a patient and hospital. Our Home Health RNs should possess strong skills in physical assessment, diagnostic interpretation, and communication to formulate an individualized plan of care according to physician orders that incorporates the analysis of initial assessment. Working alongside other nurses, Physical Therapist, Speech Therapist and or Occupational Therapist.

Responsibilities

  • Assists in development, review and revision of the plan of care.
  • Provides nursing care in accordance with physician’s plan of treatment, individual plan of care, as authorized by client’s payer source.
  • Observes, records and report's reaction to treatment and any changes in client’s condition to appropriate personnel and/or physician.
  • Provides ongoing instruction and supervision of client, family or other health team personnel in the plan of treatment.
  • Participates in education programs, quality assessment activities, nurse’s meetings and other staff meetings as required.

Requirements

  • Active RN License in the state of KS
  • Home Health experience required
  • Driver's License and Vehicle with Current Auto Insurance
  • Strong Organizational and self-management skills
  • Excellent Communication Skills
