Job Description
*Coverage is for Rutland and Bennington County 1. Actively monitors and amends patient visit schedule to maximize productivity and patient need. Takes action to account for new patients and changes in existing patient schedule, including but not limited to, calling patient to reschedule visits, reshuffling patient load to meet new demands, communicating recommended changes to supervisor, etc. 2. Performs a comprehensive (OASIS and PT) assessments of assigned patients including, but not limited to psychosocial, educational, safety and risk factor parameters, as evidenced by clinical documentation and in accordance with federal and state regulations, organizational and departmental policies and standards of care, and APTA guidelines. 3. Develops a plan of care to promote optimal functional independence and safety as documented by clinical record in accordance with federal and state regulations, organizational and departmental policies and standards of care and APTA guidelines. Creates patient specific goals and ensures that the plan of care reflects logical interventions and advancement towards these goals. 4. Plans for and provides skilled physical therapy treatment including, but not limited to, therapeutic techniques, patient/care giver education, etc. assessing the patient s status and progress on each visit and ensuring quality of care as documented in the clinical record and in accordance with federal and state regulations, organizational and departmental policies and standards of care and APTA guidelines. 5. As Care Manager and/or team member, coordinates patient care with other clinical team members, including but not limited to, physicians, nurses, care givers, other rehab services and students, ensuring optimal patient care and communication as noted in the clinical record and in compliance with federal and state regulations, organizational and departmental policies and standards of care and APTA guidelines. 6. Supervises and directs clinical activities of PTAs, PT students and home care aides in accordance with federal and state regulations, department and organizational policies and standards of care and APTA guidelines. 7. Completes and coordinates required paperwork in accordance with federal and state regulations, organizational and departmental policies, standards of care and APTA guidelines to ensure that documentation reflects care provided and the outcomes achieved as evidenced in the clinical record. 8. Completes required paperwork to ensure charging and billing processes are accurately executed, including, but not limited to, documents that support charges entered, care within insurance coverage parameters, required insurance forms completed etc. Takes action to address inconsistencies in insurance coverage and desired plan of care requirements. Notifies supervisor of issues as they arise in a timely manner. 9. Participates in in-service training programs for our customers including, but not limited to Rehabilitation, Medical, Nursing staff, volunteers, community groups, etc. 10. Maintains and enhances personal knowledge as well as promotes standards in physical therapy. 11. Proactively engages in the marketing and promotion of Rehab services including, but not limited to, developing specialized areas of service, participating in public health fairs, medical community, etc. 12. Caries out a variety of patient related health assessments including, but not limited to, PT/INR, blood pressure, etc. 13. Carries out a variety of patient related procedures including, but not limited to, staple/suture removal, wound care, etc. 14. Communicates changes in patient care schedules to scheduler and supervisor in a timely manner. 15. Completes OASIS time points in accordance with agency, State and Federal guidelines, regulations and policies. 16. Works collaboratively with QI staff to maximize accuracy of clinical notes and OASIS documents. 17. Follows agency protocol with McKesson processes, including but not limited to, locking-down visits, follow-up notes, visit completion, etc. 18. Accurately and completely fills out a variety of Agency, State and Federal forms, including but not limited to, HHABN, Medicare Notice of Non-Coverage, etc. in a timely manner. Takes action to ensure that forms are readily available during visits to facilitate timely completion. 19. Carries out visits in a time effective manner. Takes action to ensure that supervisory visits are not combined with routine visits whenever necessary. Takes action to communicate information to the Scheduler in a timely manner. 20. As necessary, assesses patients needs for HCA services, develops and coordinates POC with patient/care giver, and provides on-going supervision in accordance with State and Federal regulations. 21. Coordinates safe patient discharges and/or transfers (or upon knowledge of) and ensures all appropriate paperwork and patient information is processed in accordance with department policies and procedures, and State and Federal regulations. 22. As necessary, transmits patient incidents reports (infections, injuries, etc) to QI Specialist in a timely manner. 23. Fulfills department responsibility to participate in regular peer review audits. Provides constructive feedback and openly accepts feedback in a professional manner.
Shift: M - F 8am to 4pm with 2-3 weekend days
