Certified Pharmacy Technicians in this position will practice at the top of their certification by playing a critical role in patient safety and care coordination. Medication Historians are responsible for compiling accurate and comprehensive medication histories for patients upon admission, working directly with patients, caregivers, external pharmacies, and electronic databases.
