The Clinical Nurse Coordinator is responsible for collaborating with healthcare providers and/or pharmacists, as part of the health care team, to drive personalized health management and improve health outcomes for optimal consumers. Collects co-payments telephonically. Performs care management activities within the scope of licensure for members with complex and chronic care needs, particularly with the oncology and REMS programs. Ensures medically appropriate, high quality, cost-effective care and appropriateness of treatment and level of care. Partners with physicians and/or pharmacists to interpret appropriateness of care, intervention planning, and general clinical guidance. Collaborates with providers to assess consumer needs for early identification of and proactive planning for adverse events and continuity of care. Conducts clinical assessment to develop goals that address individual needs to develop and implement a care plan. Monitors and evaluates effectiveness of the care management plan and modifies as necessary. Minimum Requirements: Requires an ASN or ADN and a minimum of 3 years of clinical experience; or any combination of education and experience, which would provide an equivalent background. Current, valid, active unrestricted RN license in applicable state(s) required. Multi-state licensure is required if this individual is providing services in multiple states. Preferred Skills, Capabilities and Experiences: BSN preferred. Oncology experience preferred. Call center experience preferred. Experience with Enterprise Rx preferred. A&E Pharmacy is looking for a Full-Time Pharmacy Technician to join our team in our Arden Hills office. You will primarily fill medications, take compounds, package medications and enter data to record information. The Pharmacy Technician will perform functions related to the pharmacy; our company sends the medication to consumers. There is no in-person customer contact. This role requires professional judgment and extensive technical training. No experience required, we will train you! The ideal candidate should be able to work independently and have a keen eye for detail. You will be cross trained in this role to work with the daily send-out medication and with the monthly-send cycle medications. This is a Full-Time position working a rotating schedule including 9am to 5:30pm, 10am to 6:30pm, 11am to 7:30pm and 2 closing shifts of 12:30pm to 9pm as well as working 2 weekend days a month 9am to 5pm. The Clinical Nurse Coordinator is responsible for collaborating with healthcare providers and/or pharmacists, as part of the health care team, to drive personalized health management and improve health outcomes for optimal consumers.

How you will make an impact:

Primary duties may include, but are not limited to:

  • Will service patients in different states telephonically; will be on the phones majority of the day helping patients with their medications, troubleshooting any issues with medication delivery, claims, co-payments.
  • Collects co-payments telephonically.
  • Performs care management activities within the scope of licensure for members with complex and chronic care needs, particularly with the oncology and REMS programs.
  • Ensures medically appropriate, high quality, cost-effective care and appropriateness of treatment and level of care.
  • Partners with physicians and/or pharmacists to interpret appropriateness of care, intervention planning, and general clinical guidance.
  • Collaborates with providers to assess consumer needs for early identification of and proactive planning for adverse events and continuity of care.
  • Conducts clinical assessment to develop goals that address individual needs to develop and implement a care plan.
  • Monitors and evaluates effectiveness of the care management plan and modifies as necessary.

Minimum Requirements:

  • Requires an ASN or ADN and a minimum of 3 years of clinical experience; or any combination of education and experience, which would provide an equivalent background.
  • Current, valid, active unrestricted RN license in applicable state(s) required.
  • Multi-state licensure is required if this individual is providing services in multiple states.

Preferred Skills, Capabilities and Experiences:

  • BSN preferred.
  • Oncology experience preferred.
  • Call center experience preferred.
  • Experience with Enterprise Rx preferred.
  • Experience with Microsoft Office suite preferred.
  • Experience with Genesys preferred.
  • Bi-lingual or multi-language skills preferred.
