Position Summary: This is a 12-month first-year postgraduate training program. The resident will be a self-directed, independent, motivated learner guided by the Residency Program Director and the appropriate preceptors for each learning experience. The resident will rotate through 12 months of inpatient and ambulatory patient care experiences to foster independent clinical decision making through established collaborative drug therapy management agreements. Primary clinical responsibilities of the resident include rounding with assigned teams, performing pharmacokinetic monitoring, validating pharmacy orders, performing medication reconciliation, ensuring appropriate transitions of care, and providing patient education. The Pharmacy Clinical Specialist is a decentralized pharmacist position. The Pharmacist Clinical Specialist functions as a member of the multidisciplinary patient care team with direct patient care responsibilities in multiple settings across the hospital including the Intensive Care, Medicine, Surgery, Obstetrics and Nursery units, as well as the Emergency Department and Ambulatory Infusion Center. Scope of work Fills medication orders including oral, injectable and topical medications and intravenous admixtures. Assists in the bulk manufacturing of special pharmaceutical products. Compile an electronic portfolio containing a showcase of successful completion of training plan requirements and accomplishments achieved throughout internship at Meijer. This portfolio will be reviewed during quarterly conversations and interviews for Meijer Pharmacist positions. Participate in (after applicable training) Meijer Pharmacy immunization, medication therapy management, health screening and all other current clinical programs. Maintain minimum required availability for the program of 16-32 hours per month year-round as needed based on professional development and market needs. Invest as much time as needed in learning experience to ensure professional development.
