Position Summary Under direct supervision of the Health Center Manager and the administrative direction of the Pharmacist is responsible for enrolling patients in various patient assistance programs and maintains appropriate records. Essential Duties And Responsibilities Determines patient eligibility for and enrolls patients in patient assistance programs and maintains a record of such eligibility within the MAP file. Maintains tickler file for all patients that ensures that continuity of care is provided. Receives, logs and processes all MAP medications and maintains necessary logbooks, and file systems through which the MAP program is monitored and controlled. Assures that patients are charged appropriate co-pay. Logs all samples into log book upon receipt in clinic and prepares samples for dispensing by the providers. Ensures that only medications that are on the formulary are accepted. Maintains all requisite records in relation to the dispensing of samples. Responsible for maintaining medication information sheets and ensuring that these accompany each prescription. Ensures adequate supply of samples are on hand and assures quality through stock rotation and discarding of expired medications. Places orders under the direction of the Pharmacist and maintains adequate supplies of medications within the clinic. Maintains and tracks inventory control for all items within the drug room of the clinic. Completes bi-monthly inventory of pharmacy stock and reports back to the Pharmacist. Aids providers in their dispensing of in-house medication(s). Maintains crash carts and keeps requisite records. Responsible for educating the patient(s) on how to read prescription labels. File patient charts when medication has been processed. Ensure that all refills by phone, fax, or in person are complete and accurate and dispensed in a timely manner. Utilizing autogenerated reports to replenish the inpatient automated dispensing cabinets(Omnicell) with sufficient quantities of medication and supplies. Replenishing medications and supplies for inpatient units according to “on demand” orders requested by nursing staff. Conducting at least monthly inspections of all patient care units and other areas/departments of the medical center and outpatient clinic areas where medications are stored, administered, ordispensed. Maintaining proper records of medications and supplies issued to provide data for reports as needed. Utilizing the unit dose packaging equipment to prepare “unit doses” for dispensing through the Inpatient Pharmacy section. Preparing labels for repackaged medications including bar codes and specialized labeling, e.g.,half-tablet doses, oral liquid syringes, etc., in accordance with applicable laws, requirements of official packaging and established standards of good professional practice. Performing routinely scheduled preventative maintenance and cleaning of the equipment. Recording all pertinent information for each medication canister packaged. Examining prescriptions and refill requests against computer medication profiles foradministrative irregularities (i.e. missing directions, missing VA provider identification, nonformulary medication, etc.) and makes appropriate referrals to a pharmacist. Exercise good judgment regarding referral of problems requiring the professional judgment of a Clinical Pharmacist. Entering refill requests into the computer database accurately and efficiently. Removing prescription labels and related documents from computer printers and correlatesthem by patient. Selects the correct medications and counts the correct quantity when filling manually or using automated equipment when appropriate. Accurately and carefully applies labels to the prepared containers and places the completed orders in trays or bags for the final check by a Clinical Pharmacist. Verifying the identification of the patient prior to releasing the medications to the patient at pickup window. Preparing packages of completed prescriptions that are designated to be mailed andtransporting them to the appropriate area for pick-up. Operating the ScriptPro system in the filling of prescriptions and provides preventativemaintenance and cleaning of the ScriptPro filling system.
