Heritage Home Healthcare is seeking compassionate and dependable PRN caregivers in the Rio Rancho and Bernalillo, NM area to provide delegated personal care services in a home setting. In this role, you will support clients with activities of daily living, ensuring comfort, safety, and dignity. Responsibilities

• Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and eating
• Provide mobility assistance, including transfers and safe positioning
• Support medication reminders and delegated care tasks per care plan and state regulations
• Assist with light housekeeping and maintaining a safe home environment
• Provide companionship, emotional support, and encouragement
• Escort or transport clients to appointments or errands as needed
• Observe and report changes in client condition to the clinical team
• Follow all individualized care plans and instructions from licensed professionals

Qualifications

• Must be at least 18 years of age

• Valid driver's license and reliable transportation
