Overview
Job Description
- Supervise and coordinate the daily operations of a designated long-term care unit to ensure high-quality resident care.
- Lead, mentor, and evaluate RNs, LPNs, and CNAs, ensuring appropriate staffing, delegation, and performance.
- Assess residents, develop and update individualized care plans, and monitor changes in condition.
- Ensure compliance with federal, state, CMS, and facility regulations, policies, and standards of care.
- Conduct resident rounds, review clinical documentation, and ensure accurate, timely charting in the electronic medical record (EMR).
- Administer medications and treatments as needed while ensuring medication safety and regulatory compliance.
- Collaborate with physicians, nurse practitioners, therapists, dietitians, social workers, and interdisciplinary teams to coordinate comprehensive resident care.
- Investigate incidents, complete incident reports, implement corrective actions, and monitor quality improvement initiatives.
- Active RN License for NM state.
- Minimum of 1 year of RN Supervisory/Manager experience Long Term Care (LTC), Skilled Nursing Facility (SNF), or Rehabilitation experience preferred.
- Current BLS Certification (if required by the facility).
- Strong communication, teamwork, and resident care skills.
- Ability to work flexible days, including weekends and holidays as needed.
