Under the supervision of the Residential Case Manager (RCM) and the Director of Operations (DOO), the Home Coordinator is responsible for direct support of consumers in their homes and in the community as well as the smooth functioning of the home and the team. The HC helps RISE UP carry out our mission to help consumers live as independently as possible in the community, to help them to realize the fullest membership achievable in community life and to support them to the end of their life in the community setting. JOB DUTIES AND RESPONSIBILITIES: 75% Direct Consumer Support: • Plan, support and/or accompany people to be a part of all aspects of activities in their homes and communities which includes upkeep of home; grocery shopping; banking; cooking; going to church, coffee shops, health clubs, etc.; support consumers in volunteering or jobs, seeing friends, going to dances, etc. • Provide “Personal Care” -- this term refers to assistance with creating and maintaining a positive, personal appearance (clothing choices, dressing, bathing, grooming, etc.) • Help with meals (planning, shopping, food preparation.). • Assist with household needs and chores (laundry, cleaning, errands) • Assist each person to develop and maintain friendships and family relationships. Always represent self and RISE UP in a positive way. 15% Home Management • Set up rides for participants or cancel them, as necessary. • Responsible for all grocery shopping, ensuring there are fresh food and beverages (as diets will allow), and at least 2 weeks' worth of supplies always. HC schedules the delivery/pick-up of the groceries, which requires excellent schedule management. • Responsible for monitoring the paperwork in the home, such as; timesheets, MA sheets, Time Off Requests, log books, consumer information & folders, personal spending money, etc. There is to be always a stock of at least 2 weeks' worth of documents. HC will let the office know when there is less than 2 weeks' worth of documents. • Responsible for monitoring medical supplies/equipment in the home: gloves, chucks, depends, setting up medications and medication sheets with RCM. HC will alert the office before items are running low so that there are always appropriate supplies and medications in the home. • Responsible for reviewing the MAR(s) to ensure there are no errors. If there are errors noticed, the office will be notified right away. • Serve as a liaison between the office manager and the house schedules • Assist participants with all aspects of planning; appointments, birthdays, dates • Assist participants host parties, or write letters to their family/friends • Attend critical medical appointments with the RCM and take participants to non-critical appointments on their own, once they are an approved driver • HC ensures the smooth running of the home at all times – even when working elsewhere, or through thorough pre-planning of home/consumer needs prior to having time off (both scheduled & unscheduled).

Some typical responsibilities include: Ensure that the individual service plan is followed; administer or supervise treatments prescribed by physician. Make meaningful rounds to all residents, utilizing service plans; make rounds with physicians and record visits on charts. Administer all intramuscular, sub-cutaneous injections; perform all in-house testing procedures; take and record vital signs. Document acute episodes or significant changes in resident status. Work with Activity Director, DON , and Nurse Aides in carrying out the activities program. Handle the preparation and administration of prescribed medications. Order all new medications and refills; administer all medications and chart. Chart on his/her shift in accordance with facility policy, state guidelines and residents status. Supervise serving of prescribed diets and fluid intake; report persistent unresolved problems to the physician and/or DON or appropriate discipline. Timely and accurately prepare incident reports. Supervise residents who self-administer medication while following community and regulatory guidelines Provide assistance as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living. Other duties as assigned. Qualifications/Skills/Educational Requirements Requires a LPN license. Ability to lift at least 51 lbs on a regular basis and ability to help residents who require physical assistance. Fluently read, write, speak, and understand the English language. May require experience with caring for residents with memory impairment if assigned to memory care areas of a community.

Showami is a service that licensed real estate agents use to earn money by assisting with buyer and rental showings, light inspection reports, Open Houses and other real estate tasks. We are seeking licensed real estate agents in Kenosha and surrounding areas to assist with various real estate tasks. Requirements: You must be a currently licensed real estate agent ( We can help you get licensed ) You must have access to the MLS You must have experience showing homes in the Kenosha area. You must be able to show homes using an electronic lockbox Agents will be paid an average of $34 or more per task with the opportunity to receive a tip for a job well done.
