Clinicians in this role will undergo training for the model through their local VA and must pass the certification process. PCMHI clinicians provide brief, solution focused interventions for veterans. Sessions typically last 30 min or less and consist of 6 or less per issue per year. Core Responsibilities Performs initial and follow-up evaluations, including assessments. Engages the veteran, core, and expanded PACT team members in treatment planning. Provides a range of treatment services, including individual therapy. Provides evidence-based psychotherapies as necessary and appropriate. Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. About the Role:

Tidewell Hospice is seeking a Social Worker to support patients and families during their end-of-life journey. This role provides psychosocial support, care coordination, and resource assistance to ensure comfort and dignity for those we serve.

Key Responsibilities:

  • Assess patient and family needs related to coping, mental health, financial resources, and caregiver support.
  • Provide individual and family counseling, crisis intervention, and bereavement support.
  • Collaborate with the Interdisciplinary Group (IDG) to develop and manage patient care plans.
  • Assist with financial resources, Medicare/Medicaid, and long-term care planning.
  • Maintain accurate electronic medical records (EMR) and participate in IDG meetings.
  • Support patients and families with advance directives and memorial planning.
  • Educate the community on hospice care.
  • Participate in an on-call rotation.

Qualifications:

  • Education & Experience: Minimum one year of social work experience in a healthcare setting (internships may qualify).

  • Licensure: Must hold one of the following Florida licenses or License eligible (Registered Intern) within 90 days of hire:

  • Social Work (LCSW) or Social Work (RCSWI)
  • Mental Health Counselor (LMHC) or Mental Health Counselor (RMHCI)

  • Marriage and Family Therapist (LMFT) or Marriage and Family Therapist (RMFTI)

  • Other Requirements:

  • Valid Florida driver's license and reliable transportation for field-based roles.

  • Proof of valid auto insurance.
