Reporting to the Social Work Manager, the Social Worker maintains a caseload of participants working in close coordination with the IDT. In addition to delivering standard social work services such as psychosocial assessments, care planning, counseling and case management, the Social Worker functions as a liaison between the participants, family members, support network and care team, as appropriate. Essential Job Duties: Conduct Social Work assessments to determine the psychosocial needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans Deliver and document social work interventions as agreed upon in the participants’ care plans including but not limited to arranging necessary resources and services, assisting with care transitions, providing individual as well as group counseling and case management Work with the primary care physician and other members of the care team to guide smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home, etc.) Initiate, coordinate and facilitate care conference meetings to ensure the highest level of care coordination among other care team members, participants, and other people within the participants’ support network (family, informal caregivers etc.) Provide discharge planning when participants disenroll from the program Job Requirements: Master’s Degree in Social Work (MSW) required Minimum of one (1) year of experience with a frail or elderly population Previous experience coordinating and facilitating care conference meetings Previous experience assisting people with behavioral health & substance abuse issues, preferred Benefits of Working at WelbeHealth Apply your clinical expertise in new ways as we rapidly expand. This position will support implementation of a 4-year Innovation Spark grant from the Albertsons Companies Foundation to provide assistance to 200 individuals in Maryland and Illinois who are unemployed, underemployed or employed in low-wage jobs to obtain living-wage employment in the social services field. The initiative will offer 16-week job training with comprehensive support and guidance, preparing participants for jobs in the social service sector, in which there exist significant vacancies nationwide. Program participants will be trained to engage in outreach and enrollment activities for the Supplemental Nutrition Assistance Program (SNAP), Women Infants and Children Program (WIC) and other resources, thereby enhancing the grant’s impact and promoting sustainable solutions to hunger within their communities. Position is part-time. Candidates must live in or within commuting distance to Baltimore - or be willing to relocate without relocation assistance - and will spend most days working from a project office in or near the city, with the possibility of working from home on a limited basis. Family Service Association is seeking a full time (40 hours per week) Licensed Master’s level Clinician to join our Family Resource Center staff. The Clinician will conduct trauma-informed intakes, screening, and assessment functions for family members experiencing CRA-related issues.  This role is located in Fall River, MA with occasional remote option. Starting annual salary is $62,400 and the standard hours are Monday through Friday, 8:30 a.m. - 5:00 p.m. with two 10:30 am -7:00 pm shifts per week. **Some flexibility with the schedule will be necessary as this schedule will change to accommodate clients' clinical needs.  Duties and responsibilities include: Must have a willingness and ability to work with the SED, DMH and DCF populations. Arrange for the collection of relevant information regarding the family’s story, current situation, and family strengths and needs. Responsible for completing the modified CANS as well as other identified supplemental behavioral needs assessments Oversee the development of the Family Support Plan and provide ongoing supervision and case management oversight to the Family Partner. Fulfill reporting requirements of the department as well as required FRC documentation Work with all Family Members who are involved with the FRC; however, Family Members experiencing CRA-related issues are prioritized for services. We are fortunate to have significantly increased the reach of mental health care services in our communities, and look forward to continuing to provide consistent, high-quality care to those we serve for many years to come.  We will rely on your expertise to:   Evaluate mental health conditions, diagnose, create, and implement a treatment plan, complete ongoing documentation including further diagnosis, treatment plan reviews, and case notes according to company policy.  Provide excellent care for clients and collaborate with a dynamic team to further the mission of filling gaps in our community.  Utilize creativity in interventions to help clients achieve and exceed goals.   Prepare and submit individual documentation for each session per company guidelines and protocol.     For Full-Time status, clinicians must maintain at least 32 available hours for patient care per week.  Coordinate services with other care providers, families, work personnel, medical personnel, other OptiMindHealth staff, and school staff as needed.   Attend and participate in all clinical staff meetings and trainings.  Qualifications and Skill for This Positions:    Master's degree in one of the behavioral sciences or related fields from an accredited college/university.  Full, current, unrestricted license to practice independently (LPC, LCSW, LMFT) is required.  Experience with electronic health records and completing clinical documentation.   Effective written and verbal communication skills.  Experience and willingness to work collaboratively and creatively to meet necessary deadlines.  Comfort and familiarity working with clients with a diverse range of problems or concerns.    Ability to pass a background check in a way that confirms to us you can provide safe care.    Demonstrate and model stable and appropriate boundaries with clients.  Additionally Valuable (but not essential) Qualifications for This Position:    Experience working in community, group, or private practice mental health.  Currently paneled or credentialed with insurance network.  Multilingual or bilingual proficiency.  Specialty area of practice.   We can't wait to hear your story and look forward to sharing ours!   The specific statements shown in each section of this Job Description are not intended to be all-inclusive. At WelbeHealth, each participant of our program is guided by our interdisciplinary team (IDT) - composed of clinical and non-clinical members who work cohesively to provide quality comprehensive primary care and social services for frail seniors to keep them living safely in the community. Our values and participant focus lead the way no matter what. The WelbeHealth Social Worker acts as an integral part of the IDT team to help our participants achieve their full potential.

Reporting to the Social Work Manager, the Social Worker maintains a caseload of participants working in close coordination with the IDT. In addition to delivering standard social work services such as psychosocial assessments, care planning, counseling and case management, the Social Worker functions as a liaison between the participants, family members, support network and care team, as appropriate.

Essential Job Duties:

  • Conduct Social Work assessments to determine the psychosocial needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans
  • Deliver and document social work interventions as agreed upon in the participants’ care plans including but not limited to arranging necessary resources and services, assisting with care transitions, providing individual as well as group counseling and case management
  • Work with the primary care physician and other members of the care team to guide smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home, etc.)
  • Initiate, coordinate and facilitate care conference meetings to ensure the highest level of care coordination among other care team members, participants, and other people within the participants’ support network (family, informal caregivers etc.)
  • Provide discharge planning when participants disenroll from the program

Job Requirements:

  • Master’s Degree in Social Work (MSW) required
  • Minimum of one (1) year of experience with a frail or elderly population
  • Previous experience coordinating and facilitating care conference meetings
  • Previous experience assisting people with behavioral health & substance abuse issues, preferred

Salary/Wage base range for this role is $74,612 - $98,488 / year + Bonus + Equity. WelbeHealth offers competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits.  Actual pay will be adjusted based on experience and other qualifications.
