This team will partner with counties and the A4O Consultant Team to deliver results in addressing case needs across Adult & Aging Services program areas. As a member of the ISSWT, you'll engage in direct practice, including: 1. Adult Protective Services (APS) Investigate reports of abuse, neglect, and exploitation of vulnerable or disabled adults. Determine if protective services are necessary and develop service plans to ensure the safety and well-being of individuals. Coordinate with law enforcement, healthcare providers, and community partners. 2. Guardianship & Legal Interventions Initiate and manage adult guardianship proceedings when individuals are deemed incapable of making decisions. Serve as guardian of the person (and sometimes estate) if appointed by the court. Prepare legal documentation and represent DSS in court proceedings. 3. In-Home Aide and Support Services Assess client eligibility and level of need for in-home care, personal assistance, or homemaker services. Develop and monitor care plans to support adults living safely in their own homes. 4. Long-Term Care and Special Assistance Assess for placement in adult care homes or nursing facilities. Determine eligibility for Special Assistance (SA) and Medicaid-related services. Conduct annual or semiannual reassessments and reviews of placement settings. 5. Case Management and Multidisciplinary Collaboration Coordinate with mental health, public health, housing agencies, and healthcare providers. Participate in multidisciplinary teams to address complex client needs. Provide crisis intervention and long-term case planning. Determine if protective services are necessary and develop service plans to ensure the safety and well-being of individuals. Further, it is the aim of this unit is to prevent abuse, neglect and exploitation of adults but if necessary, intervene to provide protection. The primary purpose of this position in relation to Adult and Aging Services is as follows: To provide Special Assistance In-Home Services to clients that meet eligibility requirements for the program. To manage a caseload of Adult Protective Services. This shall include completing a thorough evaluation to determine if the disabled adult has been abused, neglected or exploited. The worker will also work towards mobilizing services to assist the adult is needed .To provide At-Risk Case Management Services assist adults who are at-risk or show evidence of abuse, neglect, or exploitation in gaining access to needed medical, social, educational, and other services. To manage a caseload of guardianship cases. This shall include at the minimum a quarterly contact with the ward and his/her caregiver. The social worker will have conferences with physicians and other professionals to ensure the ward's needs are being met. We are actively recruiting for Pitt County to join our Adult & Aging Services Social Work Team. This team will partner with counties and the A4O Consultant Team to deliver results in addressing case needs across Adult & Aging Services program areas.

As a member of the ISSWT, you'll engage in direct practice, including:

1. Adult Protective Services (APS)

  • Investigate reports of abuse, neglect, and exploitation of vulnerable or disabled adults.
  • Determine if protective services are necessary and develop service plans to ensure the safety and well-being of individuals.
  • Coordinate with law enforcement, healthcare providers, and community partners.

2. Guardianship & Legal Interventions

  • Initiate and manage adult guardianship proceedings when individuals are deemed incapable of making decisions.
  • Serve as guardian of the person (and sometimes estate) if appointed by the court.
  • Prepare legal documentation and represent DSS in court proceedings.

3. In-Home Aide and Support Services

  • Assess client eligibility and level of need for in-home care, personal assistance, or homemaker services.
  • Develop and monitor care plans to support adults living safely in their own homes.

4. Long-Term Care and Special Assistance

  • Assess for placement in adult care homes or nursing facilities.
  • Determine eligibility for Special Assistance (SA) and Medicaid-related services.
  • Conduct annual or semiannual reassessments and reviews of placement settings.

5. Case Management and Multidisciplinary Collaboration

  • Coordinate with mental health, public health, housing agencies, and healthcare providers.
  • Participate in multidisciplinary teams to address complex client needs.
  • Provide crisis intervention and long-term case planning.
  • Determine if protective services are necessary and develop service plans to ensure the safety and well-being of individuals.

Job Description: Social Worker III, Adult & Aging Services

The Adult and Aging Services Unit provides citizens with the social services to support the achievement of goals of self-sufficiency. Further, it is the aim of this unit is to prevent abuse, neglect and exploitation of adults but if necessary, intervene to provide protection. The primary purpose of this position in relation to Adult and Aging Services is as follows: To provide Special Assistance In-Home Services to clients that meet eligibility requirements for the program. To manage a caseload of Adult Protective Services. This shall include completing a thorough evaluation to determine if the disabled adult has been abused, neglected or exploited. The worker will also work towards mobilizing services to assist the adult is needed .To provide At-Risk Case Management Services assist adults who are at-risk or show evidence of abuse, neglect, or exploitation in gaining access to needed medical, social, educational, and other services. To manage a caseload of guardianship cases. This shall include at the minimum a quarterly contact with the ward and his/her caregiver. The social worker will have conferences with physicians and other professionals to ensure the ward's needs are being met.

KNOWLEDGE, SKILLS AND ABILITIES: Please note - all education, including high school, and all employment history should be included in the body of the application for the application to be considered complete. Thorough knowledge of social work principles, techniques, and practices, and their application to complex casework, group work, and community problems. Considerable knowledge of family and group dynamics and a range of intervention techniques, governmental and private organizations, and resources in the community, laws, regulations, and policies which govern the program. General knowledge of the methods and principles of casework supervision and training. In certain settings, considerable knowledge of medical terminology, disease processes and their treatment as they relate to decisions regarding clinical interventions and appropriate therapies based on medical or psychological diagnosis. Skill in establishing rapport with a client and in applying techniques or assessing psychosocial, behavioral, and psychological aspects of client's problems. Ability to establish and maintain effective working relationships with members of case load and their families, as well as civic, legal, medical, social, and religious organizations. Ability to express ideas clearly and concisely and to plan and execute work effectively.

MINIMUM TRAINING AND EXPERIENCE: Master's degree in social work from an accredited school of social work; Bachelor's degree in social work from an accredited school of social work. Master's degree in a human services field and one year of directly related experience*; Bachelor's degree in a human services field from an accredited college or university and two years directly related experience*; Bachelor's degree from an accredited college or university and three years of directly related experience.
