Provides guidance to members seeking alternative solutions to specific social, cultural or financial problems that impact their ability to manage their healthcare needs. Facilitates and coordinates behavioral health resources as individual member needs are identified. Establishes a relationship with member, family, physician(s), and other providers to determine use of benefits and community resources. Acts as liaison and member advocate between the member/family, physician and facilities/agencies. Evaluates members' ability to independently manage self and locate alternative resources when limitations are identified via standardized Social Work Psychosocial evaluation methods, processes and tools while maintaining accurate record of activities. Minimum Requirements: Requires a BS or MS in Social Work and a minimum of 3 years experience in case management and a minimum of 1 year experience working with aged or disabled populations; or any combination of education and experience which would provide an equivalent background. Job Overview:
As a Licensed Clinical Social Worker at Ultra Health, you will play a crucial role in providing care and support to skilled nursing facility residents. You will work collaboratively with a multidisciplinary team to deliver high-quality mental health services and will establish and build strong working relationships with administrators and staff throughout your designated location(s).
Duties:
Conduct individual therapy sessions to address a wide range of mental health concerns.
Provide Psychotherapy tailored to each client's needs.
Offer care and support for individuals experiencing trauma-related symptoms.
Collaborate with medical professionals to coordinate care for patients.
Assist in the development and implementation of treatment plans for individuals with disabilities.
Conduct assessments, create treatment goals, and maintain accurate medical records using ICD-10 coding.
Engage in Psychotherapy to support behavioral modifications and improvements.
Provide counseling services to individuals, focusing on enhancing overall well-being.
Experience:
Valid license as a Clinical Social Worker.
Experience in working with diverse populations, including geriatric patients
Proficiency in conducting individual counseling sessions.
Proficiency in Psychotherapy techniques.
Strong understanding of ICD-10 coding for accurate recordkeeping.

Minimum Qualifications

    • Active LCSW license in Tennessee
    • Experience with geriatric or adult populations
    • Skilled in psychotherapy & clinical documentation
