Your Everyday Conduct assessment of patient's psychosocial needs through intensive interviewing of patient and family members, conferring with interdisciplinary team and reviewing medical records. Evaluate coping skills, cognitive and intellectual functioning, support systems, resources, other factors that could affect responses to illness, treatment and discharge plan. Identify barriers and plans for intervention to overcome or lessen barriers to achieve outcome as evidenced by treatment plan. Communicate findings, plan to interdisciplinary team and documents assessment, plan and interventions in medical records. Provide crises intervention and supportive counseling for individuals experiencing a temporary or situational problem. Assess cases of suspected elder, child, sexual or domestic abuse or neglect. Comply with required reporting, according to state law and hospital policy. Refer patients/families to appropriate community agencies for further intervention or counseling such as palliative or hospice services as needed and facilitate family meetings to help with decision making when there is disagreement or lack of clarity around goals of care and plan of care. Facilitate interactions between staff and DCFS/EPS or other agencies. Identify patients in assigned caseload with complex social and medical issues through case finding and referral process. Review caseload with manager to share findings, needs, barriers and progress to discharge. Evaluate financial assistance needs and eligibility and directs patients/family to appropriate community agencies which can assist in meeting financial needs, or providing food, shelter, transportation or other services. Maintain a working knowledge of payor reimbursement requirements for post hospital services and community resources. Demonstrate knowledge of legal rights advance directives and patient rights with ability to counsel/educate patients/families regarding patient rights, decision making and formulating advance directives. The Justice Center’s court responds creatively to low-level offending, seeking sentences that are restorative to the defendant and community. In 2023, Midtown Community Justice Center celebrated 30 years of operation and was renamed to Midtown Community Justice Center to reflect the work we do in the community. The work has continued to move upstream and beyond the walls of our physical space through expanded diversion initiatives, street outreach, and community engagement. Midtown Community Justice Center offers a diversion program that provides people who are arrested an off-ramp from criminal prosecution: Project Reset. The Center for Justice Innovation operates Project Reset, New York City's pre-arraignment diversion program, in all five boroughs and the Midtown Community Justice Center operates the program in Manhattan. In collaboration with the District Attorney’s Offices, the Mayor’s Office of Criminal Justice, the defense bar and the New York Police Department, Project Reset seeks to create a proportionate response to low-level crime outside of the traditional criminal justice process. Project Reset-eligible community members receive the option to participate in a one session program in advance of their court date. Upon completion of the session, the District Attorney’s Office will decline to prosecute the case. The Midtown Community Justice Center also offers Rapid Reset, which offers the same diversion opportunity to Project Reset-eligible people who show up to court. Midtown Community Justice Center is seeking a Project Reset Social Worker. JOB SUMMARY To provide a safe, nurturing and stimulating environment in which the children can grow physically, socially, emotionally and intellectually using therapeutic intervention and goals of the program as guidelines. ESSENTIAL DUTIES AND RESPONSIBILITES Evaluate children in area of expertise for initial / requested / annual evaluation as per CPSE (Social Worker- Complete social history intakes.) Complete IEPs for children evaluated. Develop a weekly therapy schedule for children on caseload to meet each child’s IEP therapeutic mandates. Follow schedule for individual/small group therapy sessions. Provide and implement well planned therapeutic activities that encompass using a multitude of learning opportunities for children requiring services as recommended on the IEP. Review therapeutic goals and objectives for children to ensure continuity in therapy activities. Maintain accurate attendance records, files and paperwork (session notes, progress reports, etc.) for each child on caseload. Complete required paperwork in a timely manner (evaluations, IEP’s, quarterly progress reports, session notes, etc.). Participate in team meetings to make recommendations for development of IEP. Participate in team case conference meetings to review and update student intervention strategies. Establish and maintain professional communication with parents (notebooks, phone, etc.) keeping them informed of their child’s progress, activities, and needs. Participate in parent-teacher conferences twice yearly, or as needed, to review IEP goals/objectives and school performance. Provide and ensure appropriate supervision of children at all times. Provide and ensure appropriate, language and positive methods of disciplinary intervention as defined by our staff policy. Prepare and maintain a safe clean, and well-organized learning environment. Ensure that the Department of Health, Bureau of Day Care regulations is followed. Assist in planning school wide events. Maintain and respect confidentiality of child, parent, staff and other center issues. Maintain a high standard of classroom intervention by training classroom staff in area of expertise. Attend weekly staff meetings, in-service training and on and offsite professional development workshops, as determined by the school calendar. Keep Director informed of incidents relating to accidents, concerns about child’s progress, or extra ordinary circumstances. Appropriately represent the Center in all forums. Your Everyday

Conduct assessment of patient's psychosocial needs through intensive interviewing of patient and family members, conferring with interdisciplinary team and reviewing medical records. Evaluate coping skills, cognitive and intellectual functioning, support systems, resources, other factors that could affect responses to illness, treatment and discharge plan. Identify barriers and plans for intervention to overcome or lessen barriers to achieve outcome as evidenced by treatment plan. Communicate findings, plan to interdisciplinary team and documents assessment, plan and interventions in medical records. Provide crises intervention and supportive counseling for individuals experiencing a temporary or situational problem. Assess cases of suspected elder, child, sexual or domestic abuse or neglect. Comply with required reporting, according to state law and hospital policy. Refer patients/families to appropriate community agencies for further intervention or counseling such as palliative or hospice services as needed and facilitate family meetings to help with decision making when there is disagreement or lack of clarity around goals of care and plan of care. Facilitate interactions between staff and DCFS/EPS or other agencies. Identify patients in assigned caseload with complex social and medical issues through case finding and referral process. Review caseload with manager to share findings, needs, barriers and progress to discharge. Evaluate financial assistance needs and eligibility and directs patients/family to appropriate community agencies which can assist in meeting financial needs, or providing food, shelter, transportation or other services. Maintain a working knowledge of payor reimbursement requirements for post hospital services and community resources. Demonstrate knowledge of legal rights advance directives and patient rights with ability to counsel/educate patients/families regarding patient rights, decision making and formulating advance directives.

Master's degree Licensed Master Social Worker through the Louisiana State Board of Social Work Examiners.
