ESSENTIAL JOB FUNCTIONS / RESPONSIBILITIES: • Assesses the psychosocial status of patients related to the patient’s illness and environment per the company’s Psychosocial Assessment Policy. Issues identified related to patient coping will be included in the comprehensive assessment and addressed in the plan of care. • Provides rehabilitative and supportive social casework geared toward restoring patients to their optimum level of social and health adjustment. This includes assisting patients and their families in understanding, accepting, and following medical recommendations. • Helps patients utilize the resources of their families and the community. This may be accomplished by either referring patients to resources or acting as a liaison/advocate on behalf of the patients in their dealings with other health and welfare agencies. • Maintains a list of public and private agencies as resources for the patient and organization personnel. • Assists patients and their families with personal and environmental difficulties, which predispose them toward illness or interfere with obtaining maximum benefits from medical care. These range from counseling members of the patient’s family to assisting patients with admission into facility-based care. • Consults with physician and other members of the health team for the purpose of assisting them to understand the significant social, emotional, and environmental factors related to the patient’s health problems. • Documents all assessments, plans of care and clinical progress notes in the medical record in a timely manner • Attends case conference meetings. • Participates in staff development activities and in-service education. • Participates in the development and revision of physician’s plan of treatment for the patient. • Participates in discharge planning • Acts as consultant for other agency clinical personnel. • Communicates effectively with all providing care. • Confirms on a weekly basis the scheduling of visits with Case Manager to coordinate necessary visits with other agency clinical personnel. • Other duties as assigned by the supervisor. • Supports the mission, values and goals of Signature Healthcare at Home EDUCATION and EXPERIENCE: • Prefer Professional Social Worker Licensure in the States(s) practicing. • For Social Workers practicing in Oregon, professional license issued by the OR Board of Social Workers (LMSW or LCSW) or CSWA (Clinical Social Work Associate) is required with documentation from the OR Board of Social Work that there is an active Supervision Plan and name of Supervisor for CSWA. • Graduate of Accredited School specific to Master’s in Social Work. • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order. • Minimum of one year of experience or more, in hospital, home health, or hospice clinical setting is preferred. • Prefer experience in working in a collaborative healthcare professional team approach to care of the patient. This job engages in advanced clinical social work practice in the medical setting based on the application of social work theory, knowledge, ethics and methods to restore or enhance, social, psychosocial or bio-psychosocial functioning of individuals, couples, families, groups, organizations and communities. Applies specialized clinical knowledge and advanced clinical skills in the areas of prevention, assessment and treatment of mental, emotional, behavioral and addictive disorders; engages in multi-disciplinary treatment planning and clinical social work duties and coordinates and implements complex discharge plans through the use of the company and community-based services.


 

Education 

Required - Master's degree in Social Work (MSW)


Work Experience

Required -5 years related experience.

Preferred-Experience in a medical social work field

Certifications

Required -Licensed Clinical Social Worker (LCSW) as verified by the LABSWE and able to engage in independent practice

Knowledge Skills and Abilities (KSAs)

  • Must have computer skills and dexterity required for data entry and retrieval of patient information.
  • Must be proficient with Windows-style applications and keyboard.
  • Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
  • Good knowledge of common disease entities and disorders and medical terminology in order to understand acute and ongoing needs of patients and families
  • Excellent psychotherapeutic skills
  • Excellent verbal and written communication skills
  • Superior interpersonal skills necessary to deal effectively and courteously with patients, families, multi-disciplinary team members, departmental peers and community agency personnel.
  • Advanced knowledge of an acute and chronic care medical setting, medical terminology and the psychosocial and economic effects of illness and/or trauma upon the patient and their support system
  • Advanced knowledge of managed care and public programs such as those funded by Medicare, Medicaid, Medicare Advantage, and commercial insurance plans.


Job Duties
•  Interviews and assesses patients and assists throughout patient care plan.
•  Identifies and provides helpful resources to patients and families.
•  Maintains patient records and documents and communicates critical information to the medical team.
•  Participates in on-call rotation.
•  Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.

Physical demand requirements are in excess of those for Sedentary Work. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible. NOTE: The constant stress and strain of maintaining a production rate pace, especially in an industrial setting, can be and is physically demanding of a worker even though the amount of force exerted is negligible.

Mechanical lifting devices (carts, dollies, etc.) or team lifts should be utilized.
Must be able to sit and stand for prolonged periods of time.
Must be able to travel throughout and between facilities.
Must be able to work a flexible work schedule (e.g. 24/7, weekend, holiday, on call availability).

Normal routine involves no exposure to blood, body fluid or tissue, but exposure or potential for exposure may occur.
