About COREWELL HEALTH VNA

Our Visiting Nurses Association (VNA) is the Home Care Team serving West Michigan. We are seeking a licensed Occupational Therapist to join our growing team supporting patients across West Michigan (specifically Grandville, Holland and surrounding areas).

This role offers a high level of autonomy, allowing clinicians to practice at the top of their license while delivering care in patients' homes. Home health occupational therapy provides a unique opportunity to help patients regain independence with daily activities and safely function in the environment where they live and thrive.

Our therapists maintain manageable caseloads, typically seeing 5-7 patients per day, with thoughtfully designed geographic territories that minimize travel and support work-life balance. As part of Corewell Health West, our VNA team is committed to delivering exceptional, patient-centered care across Ottawa County and surrounding communities.

Scope of Work

Provides therapy to patients based on patient need and medical direction. Provides patient/caregiver education and arranges for adaptive equipment as needed.

Standard Qualifications

• Required Bachelor's Degree or equivalent from an accredited occupational therapy program
• Preferred Master's Degree
• LIC-Occupational Therapist- STATE_MI State of Michigan Upon Hire required
• CRT-Occupational Therapist, Registered (OTR) - NB-COT National Board for Certification in Occupational Therapy Upon Hire required
• LIC-Driver's License- STATE_MI State of Michigan Home Care Business Lines Upon Hire required
• CRT-Basic Life Support (BLS) - AHA American Heart Association 90 Days required Or
• CRT-Basic Life Support (BLS) - ARC American Red Cross 90 Days required
• CRT-Hand Therapist, Certified (CHT) - HTCC Hand Therapy Certification Commission Hand Therapy Program Upon Hire preferred
