University of Colorado Anschutz Medical Campus Department: Pediatric Cardiology Job Title: Single Ventricle APP Position #: 00675964 - Requisition #:37631 Job Summary: The Single Ventricle APP (SV APP) will be part of the multidisciplinary team responsible for managing and coordinating the care of the single ventricle patient population within our Heart Institute. The SV APP will partner and collaborate with out SV director for ongoing single ventricle programmatic needs. The SV APP will consult and facilitate coordination of care with the inpatient cardiology teams. The SV APP would provide continuity of care for single ventricle patient needs and function as a direct point of contact for families throughout the spectrum of single ventricle care. Key Responsibilities New and follow-up clinic for CCHD and SVCC patients Inpatient single ventricle interstage consults Emergency add-on visits for CCHD patients (ie: red flag visits) Admission/discharge support for single ventricle patients Triaging calls/pages from patient/families Ordering of diagnostic labs, diagnostic or interventional procedures, surgical interventions Medication management and monitoring Management and monitoring of growth/nutritional needs Facilitate communication between SV program and referring providers Telehealth visits for out of state patients (New Mexico, Wyoming, Western Kansas, Montana) Discharge care coordination and education Involvement in multicenter collaboratives including the Fontan Outcomes Network, the Pediatric Heart Network, Cardiac Neurodevelopmental Outcome Collaborative, and the National Pediatric Cardiology Quality Improvement Collaborative Data base management Work Location: Onsite - this role is expected to work onsite and is located in Aurora, CO. Job Summary:

The Single Ventricle APP (SV APP) will be part of the multidisciplinary team responsible for managing and coordinating the care of the single ventricle patient population within our Heart Institute. The SV APP will partner and collaborate with out SV director for ongoing single ventricle programmatic needs. The SV APP will consult and facilitate coordination of care with the inpatient cardiology teams. The SV APP would provide continuity of care for single ventricle patient needs and function as a direct point of contact for families throughout the spectrum of single ventricle care.

Key Responsibilities

  • New and follow-up clinic for CCHD and SVCC patients
  • Inpatient single ventricle interstage consults
  • Emergency add-on visits for CCHD patients (ie: red flag visits)
  • Admission/discharge support for single ventricle patients
  • Triaging calls/pages from patient/families
  • Ordering of diagnostic labs, diagnostic or interventional procedures, surgical interventions
  • Medication management and monitoring
  • Management and monitoring of growth/nutritional needs
  • Facilitate communication between SV program and referring providers
  • Telehealth visits for out of state patients (New Mexico, Wyoming, Western Kansas, Montana)
  • Discharge care coordination and education
  • Involvement in multicenter collaboratives including the Fontan Outcomes Network, the Pediatric Heart Network, Cardiac Neurodevelopmental Outcome Collaborative, and the National
  • Pediatric Cardiology Quality Improvement Collaborative
  • Data base management

Onsite - this role is expected to work onsite and is located in Aurora, CO.

The Society of Thoracic Surgery requires a hospital to perform at least 250 heart surgeries per year to be considered a high-volume center. By far exceeding this, we gain more experience, which leads to better outcomes. The Heart Institute has 80 plus faculty members and continues to grow annually.

This is an open rank position and could be categorized as Instructor of Clinical Practice or Senior Instructor of Clinical Practice based on experience and qualifications as indicated below:

Instructor of Clinical Practice:

  • Certified Nurse Practitioners are certified advanced practice nurses who provide primary health care and specialized health services. Physicians Assistants are licensed to provide primary health care and specialized health services. First time appointees must complete the application process to receive authorization to provide services as an allied health professional.
  • Current Colorado license as a Registered Nurse, including registration in the advanced practice nurse registry or current license as a Certified Physician Assistant.
  • Education: Graduation from a master's program in nursing accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing (NLN), or, graduation from an accredited Physician Assistant Program.
  • Completion of a formal postgraduate nurse practitioner track or program in the applicant's specialty within a school of nursing granting graduate level academic credit, or completion of a formal Physician Assistant program with evidence of successful completion either through a certificate or a letter from the program director.
  • Compliance with the requirements under the Colorado Nurse Practice Act (for NP candidates only).
  • Documentation of prescriptive authority granted by the Colorado Board of Nursing. Nurse Practitioners who have not been granted prescriptive authority by the Colorado Board of Nursing must obtain prescriptive authority by the Colorado Board of Nursing within two appointment cycles for full time clinical FTE.
  • A DEA license is required to prescribe any medications. Medications cannot be prescribed in the electronic health record until the DEA license is obtained.
  • Current certification in basic life support (BLS) and ability to obtain pediatric advanced life support (PALS), and ACLS.

Senior Instructor of Clinical Practice:

Preferred Qualifications:
