Job Overview: The Baylor Registered Nurse (RN) plays a vital role in delivering high-quality, person-centered home health care services across Boston and its immediate surrounding areas (Braintree, Quincy, Everett, Revere ) during weekend operations. This position functions independently, providing clinical excellence and ensuring clients remain safe, healthy, and supported in their homes while reducing preventable hospitalizations and re-hospitalizations. This is a part-time weekend salaried role that requires a minimum of two weekends per month, with compensation equivalent to 24 hours paid for 16 hours of work. Essential Functions: Function independently during weekend Baylor shifts, working 16 hours every Saturday and Sunday (paid for 24 hours) to deliver skilled nursing services throughout Boston and its surrounding areas (Everett, Revere, Quincy, Braintree) . Complete intake and annual assessment and admission visits. Complete all required documentation including client assessment forms and multidisciplinary plan of care and communicate professionally with client PCP and other healthcare providers when necessary. Review all information in the client's admission binder, ensuring understanding as well as educate the caregiver on use of the MH required caregiver communication tool or electronic visit verification system depending on program participation. Complete regular, in-home visits, caregiver education/skills training and assess health and environmental changes. Conduct semi-annual visits, reviewing and updating existing plan of care and corresponding goals, medication review and reconciliation and review caregiver communication tools. Address concerns regarding environment, psychosocial status and changes in physical health Participate in multidisciplinary team meetings and work collaboratively to address client needs. Foster culture of customer service and commitment to quality care. Serve as a brand ambassador for VITRA reflecting our vision, mission, and values. Show genuine interest and compassion and commitment to the diversity of our clients and team members. Other duties as assigned. Physical and Environmental Demands: Works in a clean, well-lit environment with fluctuating temperatures in close proximity to others. Requires substantial periods of repetitive work utilizing a computer, monitor, keyboard, and mouse. Requires lifting and carrying equipment and supplies weighing up to 35 pounds, requires frequent sitting more than 75% of the workday, requires the ability to navigate stairs; requires visual acuity and manual dexterity to operate equipment.
